THE CAMDEN COALITION OF HEALTHCARE PROVIDERS (CAMDEN COALITION) EXISTS TO SPARK A FIELD AND MOVEMENT THAT UNITES COMMUNITIES OF CAREGIVERS IN CAMDEN AND ACROSS THE NATION TO IMPROVE THE WELLBEING OF INDIVIDUALS WITH COMPLEX HEALTH AND SOCIAL NEEDS. CAMDEN, NJ IS A CITY OF OVER 75,000 RESIDENTS THAT IS PLAGUED BY SOME OF THE NATION'S HIGHEST RATES OF POVERTY AND VIOLENCE. CURRENTLY ONE PERCENT (1%) OF PATIENTS IN CAMDEN ACCOUNT FOR NEARLY 30% OF CITY RESIDENTS' HOSPITAL COSTS. AS A COALITION OF HEALTH CARE PROVIDERS, COMMUNITY PARTNERS, AND ADVOCATES, CAMDEN COALITION IS COMMITTED TO ELEVATING THE HEALTH OF PATIENTS FACING THE MOST CHALLENGING MEDICAL AND SOCIAL COMPLEXITIES WITHIN THE CITY OF CAMDEN AND SURROUNDING REGION. CAMDEN COALITION DESIGNS, IMPLEMENTS, AND TESTS INNOVATIVE HEALTH CARE DELIVERY MODELS, WHICH INCLUDES, BUT IS NOT LIMITED TO, CARE COORDINATION AND ENHANCED PRIMARY CARE PRACTICES. CAMDEN COALITION BELIEVES THAT DATA, ENGAGEMENT, AND REDESIGN ARE ESSENTIAL TO REVITA

National Center for Complex Health and Social Needs-The National Center for Complex Health and Social Needs aims to improve the outcomes for people with complex medical, behavioral, and social needs. It works to coalesce a new field of care by bringing together a broad range of clinicians, researchers, policymakers, and consumers who are developing, testing, and scaling new models of team-based, integrated care. The Center and its staff collaborate with other experts across the nation to advance best practices, inform policy and research, and foster an engaged community to develop this work and teach it to others.
Population Health Initiatives- The Camden Coalitions two-pronged approach, consisting of patient and system engagement, is necessary to improve the quality of care for individuals experiencing complex health and social needs. Population Health Initiatives administers high touch care management interventions and manages the operations of the Camden Coalition's Medicaid Accountable Care Organization (ACO). The ACO aims to deliver more coordinated, efficient , and effective care to 60,000 residents of Camden, which includes many of the city's most vulnerable citizens. The Camden ACO, along with ACOs in Trenton and Newark, are evolving into what are being by bringing together area hospitals, behavioral health providers, community residents, and organizations, we aim to highlight and close gaps in the healthcare system where vulnerable citizens could adequately get their needs met.
Integrated Data and Information Systems- The Coalition works to break down informational silos within the healthcare system and across sectors by sharing, analyzing, and evaluating quantitative and qualitative data. Our integrated data framework highlights ways to improve upon our interventions and the complex systems (e.g., medical, behavioral, and social services) with which our patients interact. The Coalition runs a healthcare information exchange and an integrated database that brings together data from health care, criminal justice, school district, etc. sources.
