RESIDENTAL SERVICES: PFP'S RESIDENTIAL SERVICES PROVIDES MANY CHOICES IN LIVING ARRANGEMENTS FOR INDIVIDUALS WITH DISABILITIES OR OTHER CHALLENGES. SUPERVISON CAN RANGE FROM AS LITTLE AS TWO HOURS A WEEK TO 24 HOURS A DAY. THE DECISION OF WHERE TO LIVE REFLECTS THE LIFE PLAN THAT HAS BEEN DEVELOPED IN COLLABORATION WITH THE INDIVIDUAL, THEIR FAMILY AND/OR OTHER INTERESTED PEOPLE WHO ARE PART OF THE INDIVIDUAL'S LIFE. PFP PRIDES ITSELF ON ITS TALENTED AND PROFESSIONAL STAFF WHICH PROVIDES A CAREFUL BALANCE OF INDEPENDENCE AND MONITORED CARE TO OUR RESIDENTS. 32 INDIVIDUALS WERE SERVED IN RESIDENTIAL SERVICES DURING FISCAL YEAR.
COMMUNITY BASED SUPPORTS: PFP'S COMMUNITY BASED SUPPORTS ARE SERVICES THAT PROVIDE DIRECT SUPPORT AND ASSISTANCE FOR PARTICIPANTS, IN OR OUT OF THE PARTICIPANT'S RESIDENCE TO ACHIEVE AND MAINTAIN THE OUTCOMES OF INCREASED INDEPENDENCE, PRODUCTIVITY, ENHANCED FAMILY FUNCTIONS, AND INCLUSION IN THE COMMUNITY. COMMUNITY-BASED SUPPORTS ARE DELIVERED ONE-ON-ONE WITH A PARTICIPANT AND INCLUDE ASSISTANCE WITH COMMUNITY-BASED ACTIVITIES AND ASSISTANCE TO, AS WELL AS TRAINING AND SUPERVISION OF, INDIVIDUALS AS THEY LEARN AND PERFORM THE VARIOUS TASKS THAT ARE INCLUDED IN BASIC SELF-CARE, SOCIAL SKILLS, AND ACITIVITES OF DAILY LIVING. 35 INDIVIDUALS WERE SERVED IN COMMUNITY BASED SERVICES DURING THE FISCAL YEAR.
SUPPORTED EMPLOYMENT: PFP'S SUPPORTED EMPLOYMENT PROGRAM OFFERS CREATIVE EMPLOYMENT SOLUTIONS TO RECRUITING, TRAINING AND CONSULTING CONCERNS. THE LEVEL OF COMMITMENT PFP OFFERS IS UNIQUE, BY OFFERING JOB DEVELOPMENT, JOB COACHING, AND LONG-TERM FOLLOW ALONG (LTFA). INDIVIDUALS ARE ASSITED WITH CREATING A RESUME/COVER LETTER, NETWORKING WITH EMPLOYERS, COMPLETING JOB APPLICATIONS, AND PREPARING FOR INTERVIEWS. PFP'S CERTIFIED JOB COACHES SUPPORT INDIVIDUALS IN LEARINING JOB TASKS, COMMUNICATING PROFESSIONALY WITH CO-WORKERS, AND FINDING THE BEST TRANSPORTATION AND LEARING ROUTE TO AND FROM THE JOB SITE. LTFA IS PROVIDED TO ASSURE INDIVIDUALS ARE MAITIANING JOB STABILIZATION AND THE ENGAGEMENT OF NATURAL SUPPORTS. 21 INDIVIDUALS WERE SERVED IN SUPPORTED EMPLOYMENT DURING THE FISCAL YEAR.
