TO PROMOTE AND IMPROVE THE QUALITY OF LIFE AND TO ELIMINATE FACTORS LEADING TO POVERTY FOR NORTHERN NEW JERSEY RESIDENTS BY PROVIDING COMPREHENSIVE HEALTH AND SOCIAL SERVICES WITH COMPASSION, THROUGH COLLABORATIONS AND PARTNERSHIPS IN A CULTURALLY SENSITIVE ENVIRONMENT.
HEALTHCARE SERVICES: HEALTH CARE SERVICES ARE PROVIDED FROM 10 SITES STRATEGICALLY LOCATED THROUGHOUT HUDSON, BERGEN AND PASSAIC COUNTIES. THESE SERVICES INCLUDE PRIMARY AND PREVENTIVE HEALTH CARE FOR BOTH THE UNINSURED AND UNDER-INSURED PREDOMINANTLY MINORITY RESIDENTS, AND COVERS PRENATAL, GYNECOLOGICAL, FAMILY PLANNING, PEDIATRICS, AND ADULT MEDICINE AS WELL AS COMPREHENSIVE ORAL HEALTH CARE, MENTAL HEALTH, AND ADDICTION SERVICES COUNSELING. THE AGENCY CONTINUES TO FOCUS ON REDUCING UNNECESSARY HOSPITALIZATIONS AND USE OF EMERGENCY ROOMS. IN 2019, 72,806 UNDUPLICATED PATIENTS WERE ASSISTED THROUGH 271,605 PROFESSIONAL VISITS. EIGHTY-ONE PERCENT (81%) OF PATIENTS ARE LATINO AND 70% ARE SUFFICIENTLY LIMITED IN THEIR ENGLISH THAT THEY WERE SERVED IN ANOTHER LANGUAGE. EIGHTY-NINE PERCENT (89%) HAD INCOMES AT OR BELOW 200% OF THE FEDERAL POVERTY LEVEL (FPL); AND OF THAT NUMBER, 71% FALL AT OR BELOW 100% FPL. UNINSURED PATIENTS MADE UP 45% OF THE PATIENTS, AND 42% WERE COVERED BY MEDICAID; THIS INCLUDES 15,509 CHIP. EARLY CHILDHOOD DEVELOPMENT AND CHILD CARE: FUNDED TO SERVE 367 PRE-SCHOOL CHILDREN, 104 INFANTS AND TODDLERS, AND 30 PREGNANT WOMEN, THE NORTH HUDSON COMMUNITY ACTION (NHCAC) HEAD START/EARLY HEAD START (HS/EHS) PROGRAMS PROVIDE FULL-DAY, FULL-YEAR EARLY EDUCATION AND PARENTING PROGRAMS, INTEGRATED WITH EXTENDED CHILDCARE, HEALTH, AND FAMILY SUPPORT SERVICES. 93% OF FAMILIES SERVED THIS FISCAL YEAR HAD INCOMES AT OR BELOW 100% OF POVERTY AND 192 CHILDREN PARTICIPATED IN PRE-SCHOOL ACTIVITIES AND ARE DEVELOPMENTALLY READY TO ENTER KINDERGARTEN.
COMMUNITY SERVICES: ENCOMPASSES ALL PROGRAMS NOT INCLUDED IN THE FOLLOWING CATEGORIES.WOMEN, INFANTS & CHILDREN (WIC) NUTRITIONAL PROGRAM - WIC NUTRITIONAL EDUCATION AND ASSISTANCE SERVICES CONTINUES TO PROVIDE SUPPLEMENTAL NUTRITIOUS FOODS TO PREGNANT, BREASTFEEDING AND POSTPARTUM WOMEN, INFANTS AND CHILDREN UP TO THE AGE OF FIVE. THE PROGRAM ALSO INCLUDES NUTRITION EDUCATION AND COUNSELING, BREASTFEEDING PROMOTION AND SUPPORT, IMMUNIZATION SCREENING AND HEALTH CARE AND SOCIAL SERVICE REFERRALS, AS WELL AS VOUCHERS. THESE VOUCHERS ARE REDEEMABLE FOR SPECIFICALLY DESIGNATED FOODS AT PARTICIPATING SUPERMARKETS. NORTH HUDSON COMMUNITY ACTION CORPORATION IS A HUD APPROVED HOUSING COUNSELING AGENCY.TOTAL CLIENTS SERVED: 193HUD COC RAPID RE-HOUSING - PROVIDES QUICK ENTRY INTO HOUSING PAIRED WITH TEMPORARY FINANCIAL ASSISTANCE AND INDIVIDUALIZED CASE MANAGEMENT THAT ASSISTS A MINIMUM OF 22 HOUSEHOLDS WITH THEIR HOUSING STABILITY GOALS.
