Organizations Filed Purposes:
BHNNY was created to participate in New York States Delivery System Reform Incentive Payment Program (DSRIP) as the lead entity of a Performing Provider System serving Albany, Columbia, Greene, Saratoga, and Warren Counties and to implement the DSRIP Project Plan submitted and approved by the New York State Department of Health. The goals of the DSRIP Project Plan are to create and sustain an integrated healthcare delivery system that can meet the needs of Medicaid and other eligible patients in an effective and efficient manner. BHNNY works collaboratively with Partner Organizations to improve care, care access, and reduce avoidable and preventable utilization of the Emergency Department and In-patient hospitalizations, and ultimately reduce costs.
