The Bronx Health Link, Inc. is dedicated to creating linkages between different providers, coalitions and organizations to improve community health services
Maternal and Infant health program - Main purpose is to improve health equity and maternal and infant health outcomes. TBHL delivers intensive interventions to the Bronx neighborhoods with the greatest inequities in maternal and infant health outcomes. TBHL coordinates and conducts workshops on breastfeeding, safe sleep, women's health and family planning directly to reproductive age women and their families.
Healthy Start - main goal is to strengthen health education among high-risk women about family planning, health and mental health care, and social services for high-risk women and families.
Community Health Literacy ("CHL")- The CHL program is designed to develop patient health literacy and healthcare system navigation skills of under-utilizers of healthcare services and effectively engage or re-engage them in primary care, health home and social services as needed. Through outreach, the provision of basic CHL education, and engagement, TBHL seeks to cultivate a more informed, activated community in which individuals are enabled to act as partners in their healthcare.
