PRIMARY CARE - PERINATAL, PEDIATRIC, ADOLESCENT, ADULT, GERIATRIC, OB/GYN, DENTAL, HIV TESTING AND TREATMENT SUPPORT-COUNSELING, CASE MANAGEMENT, HEALTH EDUCATION, NUTRITION, WOMEN, INFANT, AND CHILDREN ("WIC").
HEALTH CENTER: BEDFORD STUYVESANT FAMILY HEALTH CENTER (BSFHC) IS A NETWORK OF FEDERALLY QUALIFIED COMMUNITY HEALTH CENTERS BASED IN BEDFORD STUYVESANT BROOKLYN, NY. OUR MISSION IS TO PROVIDE THE MOST PROFESSIONAL, COURTEOUS AND HIGHEST QUALITY HEALTH CARE, WITH DIGNITY, TO THOSE WE SERVE, ESPECIALLY THE UNDERSERVED POPULATION, WITHOUT REGARD FOR THEIR ABILITY TO PAY. IN 2019, BSFHC SERVED 15,964 PATIENTS THROUGH 60,882 PATIENT VISITS AT TWO CLINICS AND THREE SCHOOL-BASED CLINICS. AS A FEDERALLY QUALIFIED HEALTH CENTER, BSFHC PLAYS A VITAL ROLE IN PROVIDING HEALTH SERVICES TO POOR AND MEDICALLY UNDERSERVED POPULATIONS WHO OFTEN HAVE DIFFICULTY ACCESSING QUALITY HEALTH CARE.
WIC PROGRAM:THE BED-STUYESANT FAMILY WIC IS A FEDERALLY AND STATE FUNDED PROGRAM WHICH PROVIDES APPROPRIATE HEALTHY SUPPLEMENTAL FOODS AND NUTRITION AND BREASTFEEDING EDUCATION AND SUPPORT FOR PREGNANT WOMEN, NEW MOTHERS, INFANTS AND CHILDREN UNDER AGE FIVE. WIC HAS A PROVEN TRACK RECORD OF POSITIVE HEALTH OUTCOMES AND REFERS A LARGE NUMBER OF PARTICIPANTS FOR HEALTH CARE AND OTHER RELEVANT SERVICES WHICH BENEFIT THEM. OUR WIC PROGRAM IS TO IMPROVE THE HEALTH OUTCOME OF LOW-INCOME WOMEN, INFANTS AND CHILDREN UP TO 5 YEARS WHO ARE AT NUTRITION RISK. WE MAKE THIS POSSIBLE BY PROVIDING CULTURALLY SENSITIVE PARTICIPANT-CENTERED NUTRITION AND BREASTFEEDING EDUCATION AND SUPPORT AS WELL AS APPROPRIATE NUTRITIOUS FOODS TO SUPPLEMENT THE PARTICIPANTS' DIETS. WE PROMOTE A HEALTHY LIFESTYLE WITH INTERACTIVE ENCOUNTERS SUCH AS OUR CHILDREN'S CHEF ACADEMY FOR YOUNG CHILDREN, 3-5 YEARS. WE ALSO PROMOTE AND SUPPORT BREASTFEEDING THROUGH OUR AWARD WINNING BREASTFEEDING TEAM (2017 LOVING SUPPORT AWARDS OF EXCELLENCE, GOLD, FROM USDA). IN 2019, WE SERVED OVER 2,300 PARTICIPANTS.
MSA WELLNESS PROGRAM:THE WELLNESS CENTER PROGRAM IS ORGANIZED TO FACILITATE HIV AND HEPATITIS C CARE, TREATMENT AND PREVENTION SERVICES FOR PATIENTS OF THE BEDFORD-STUYVESANT FAMILY HEALTH CENTER (BSFHC). THE PROGRAM'S MEDICAL CASE MANAGERS MEET WITH BSFHC CLINICIANS AND OTHER PROVIDERS TO ESTABLISH PATIENT TREATMENT PLANS INTENDED TO SUPPORT, WITH THE MANAGEMENT OF POTENTIAL BARRIERS THAT COULD NEGATIVELY IMPACT ADHERENCE TO CARE. THE PROGRAM ALSO INCREASES HEALTH LITERACY AND ACCESS TO CARE FOR RESIDENTS IN THE BEDFORD-STUYVESANT NEIGHBORHOOD AND NEARBY JURISDICTIONS THROUGH OUTREACH, COMMUNITY ENGAGEMENT ACTIVITIES, AND THE DELIVERY OF HEALTH EDUCATION SESSIONS. DURING THE 2019 YEAR, THE MEDICAL CASE MANAGEMENT PROGRAM SUPPORTED 129 UNIQUE PATIENTS THROUGH 1,522 ENCOUNTERS, AND DELIVERED 4,329 SERVICES. AS WELL, THROUGH OUTREACH ACTIVITIES, THE PROGRAM HAD 3,179 ENCOUNTERS THROUGH WHICH INDIVIDUALS WERE REFERRED TO THE HEALTH CENTER FOR HIV, HEP C AND PREP/PEP CARE, INSURANCE NAVIGATION AND OTHER RESOURCES.
