THE SERVICES INCLUDE YOUTH SERVICES, ADULT SERVICES, GAY/BISEXUAL/TRANSGENDER/MSM (MEN WHO HAVE SEX WITH MEN) SERVICES, AND PREVENTION AND SUPPORT SERVICES FOR WOMEN. BAC PROVIDES INNOVATIVE, CUTTING EDGE, FUN EDUCATIONAL WORKSHOPS. THE PROGRAMS ARE CULTURALLY SENSITIVE, AGE APPROPRIATE, AND SUITED PERFECTLY FOR ANY AUDIENCE OF ANY SIZE.
BEHAVIORAL HEALTH- BRIDGING ACCESS TO CARE,INC (BAC) OPERATES AN OFFICE OF MENTAL HEALTH (OMH) LICENSED OUTPATIENT CLINIC OFFERING COMPREHENSIVE SERVICES TO CLIENTS WHO HAVE A RANGE OF MENTAL HEALTH PROBLEMS. THE PROGRAM'S OBJECTIVE IS TO IMPROVE HEALTH OUTCOMES THROUGH SYMPTOM MANAGEMENT/STABILIZATION. THE SERVICES PROVIDED INCLUDE: COMPREHENSIVE PSYCHIATRIC AND PSYCHO SOCIAL EVALUATIONS, INDIVIDUAL AND GROUP PSYCHOTHERAPY, MEDICATION MANAGEMENT, HOME & COMMUNITY BASED SERVICES, AND OTHER BEHAVIORAL HEALTH SUPPORT SERVICES. BAC ALSO OPERATES AN OASAS-LICENSED OUTPATIENT SUBSTANCE USE TREATMENT PROGRAM. THIS CLINIC OFFERS A PERSON-CENTERED TREATMENT MODEL WITH A PSYCHODYNAMIC AND COGNITIVE-BEHAVIORAL APPROACH, AND PROVIDES COMPREHENSIVE ENGAGEMENT, ASSESSMENT, EDUCATION, AND COUNSELING SERVICES (BOTH INDIVIDUAL AND GROUP).
SUPPORTIVE HOUSING- BRIDGING ACCESS TO CARE, INC ASSISTS HOMELESS AND MARGINALLY-HOUSED INDIVIDUALS LIVING WITH HIV/AIDS AND/OR WITH HISTORIES OF SUBSTANCE USE THROUGH ITS SCATTER-SITE HOUSING PROGRAMS. THESE SERVICES INCLUDE: HOUSING PLACEMENT ASSISTANCE PROGRAM, LIVING SKILLS GROUPS, PERMANENT SUPPORTIVE HOUSING, SCATTER-SITE HOUSING, AND HOUSING AND SUPPORTIVE SERVICES ON A LONG-TERM BASIS FOR HOMELESS INDIVIDUALS WITH DISABILITIES.
