THIS PROGRAM PROVIDES THE LOCAL/STATE GOVERNMENT A COST EFFECTIVE SERVICE FOR THE PERSON WITH A DISABILITY EMPOWERING THE CONSUMER WITH CONTROL OF THEIR OWN CARE, AND PROVIDING THE LOCAL AND STATE GOVERNMENT A FISCAL INTERMEDIARY.

ERIE COUNTY DEPT. OF MENTAL HEALTH CONTRACT - THE EXEMPT PURPOSE ACHIEVEMENT OF THE ERIE COUNTY DEPT. OF MENTAL HEALTH CONTRACT IS TO WORK WITH CONSUMERS WHICH HAVE SERIOUS AND PERSISTENT MENTAL ILLNESS TO ASSIST IN THE TRANSITION FROM THE HOSPITAL TO THE COMMUNITY AND TO PROVIDE COMMUNITY AND EMPLOYMENT SUPPORT. THE PROGRAM PROVIDED PEER COUNSELING, INDEPENDENT LIVING SKILLS, ADVOCACY, EMPOWERMENT SKILLS, AND SUPPORT THAT PROVIDES DE-INSTITUTIONALIZATION.

ADULT CARE & CONTINUING EDUCATION SERVICE - VOCATIONAL REHABILITATION - INDEPENDENT LIVING - THE EXEMPT PURPOSE ACHIEVEMENT OF THE ADULT CARE & CONTINUING EDUCATION SERVICE - VOCATIONAL REHABILITATION IS TO PROVIDE THE CORE SERVICES OF PEER COUNSELING, INDEPENDENT LIVING SKILLS, ADVOCACY, AND INFORMATION AND REFERRALS TO PERSONS WITH DISABILITIES. THESE PROGRAMS ARE TO PROVIDE SUPPORT AND EDUCATION FOR PEOPLE WITH DISABILITIES TO TAKE CONTROL OF THEIR OWN LIVES AS THEY WORK AND LIVE IN THE COMMUNITIES OF THEIR OWN CHOICE. THE FUNDING ALSO PROVIDES THE MANDATE TO WORK ON COMMUNITY CHANGE PROJECTS THAT MAKE THE LOCAL COMMUNITY MORE ACCESSIBLE FOR PERSONS WITH DISABILITIES BY PROVIDING CONSULTATION ON ATTITUDINAL, COMMUNICABLE, AND PHYSICAL BARRIERS THAT EXIST.
