Organizations Filed Purposes: ELLENVILLE REGIONAL HOSPITAL PROVIDES EXCEPTIONAL HEALTH CARE SERVICES TO ALL PEOPLE WHO LIVE IN, WORK IN AND VISIT OUR SURROUNDING COMMUNITIES. THIS HEALTH CARE IS DELIVERED WITH COMPASSION AND RESPECT BASED ON OUR COMMITMENT TO IMPROVING OUR COMMUNITY HEALTH THROUGH EXCELLENCE, INNOVATION AND STATE-OF-THE-ART TECHNOLOGIES.

IN-PATIENT DEPARTMENTS (MED/SURG AND SWING REHAB) & AMBULATORY SURGERY (OR): IN 2019, THE HOSPITAL AND THE FOUNDATION SUPPORTED OUR STAFF WITH TUITION ASSISTANCE AND REIMBURSEMENT FOR EDUCATION RELATED TO HEALTH CARE SERVICES. THIS GENEROUS ASSISTANCE MADE IT POSSIBLE FOR THREE OF OUR STAFF MEMBERS TO PURSUE HIGHER DEGREES IN HEALTH CARE RELATED FIELDS. (SEE CONTINUATION ON SCHEDULE O)ERH TAKES PRIDE IN SERVING AS A TEACHING HOSPITAL AND HOSTING STUDENTS FROM MANY SCHOOLS SUCH AS SULLIVAN COUNTY COMMUNITY COLLEGE, ULSTER COMMUNITY COLLEGE, BOCES NEW VISIONS PROGRAM, LEMOYNE, TOURO MEDICAL COLLEGE, CLARSON, UNIVERSITY OF BUFFALO, ALBANY COLLEGE OF PHARMACY, RHODE ISLAND COLLEGE OF PHARMACY, ANTIGUA MEDICAL UNIVERSITY AND MARIA COLLEGE. ONCE AGAIN, THE COHORT OF 119 STUDENTS BENEFITED FROM WORKING SIDE BY SIDE WITH OUR SKILLED AND COMPASSIONATE CLINICAL TEAM. DURING THEIR CLINICAL ROTATIONS, THEY OBSERVED FIRSTHAND THE DELIVERY OF EXCEPTIONAL CARE IN BOTH OUR INPATIENT UNIT AND EMERGENCY DEPARTMENT. ERH RECEIVED NO FUNDING FOR PROVIDING THESE EDUCATIONAL OPPORTUNITIES. ERH'S SUB ACUTE REHABILITATION UNIT SUCCESSFULLY TRANSITIONED PATIENTS' BACK HOME AVOIDING PLACEMENT IN TO LONG-TERM CARE FACILITIES. ONLY ABOUT 5% OF THIS COHORT OF PATIENTS REQUIRED PLACEMENT.PATIENT SATISFACTION AND POSITIVE OUTCOMES ARE BOTH EXCELLENT MEASURES FOR QUALITY. IN 2019, PATIENT SATISFACTION SCORES REACHED 96% FOR THE INPATIENT SERVICES. OUR RETURN TO HOME RATE FROM SUB-ACUTE REHAB WAS AT 81.38%. THESE STELLAR RESULTS ARE THE PRODUCT OF A MULTIDISCIPLINARY COLLABORATIVE EFFORT AMONG NURSING, PHYSICAL THERAPY, OCCUPATIONAL THERAPY, SPEECH-LANGUAGE PATHOLOGY, SOCIALIZATION ACTIVITIES, AND A REGISTERED DIETITIAN. THE LEFT VENTRICULAR ASSIST DEVICE (LVAD) PROGRAM INITIATED AT ERH IN EARLY 2016 IN COLLABORATION WITH WESTCHESTER MEDICAL CENTER CONTINUES STRONG. IN 2019, WE CONTINUED TO SERVE LVAD PATIENTS AND THEIR FAMILIES IN OUR SUB ACUTE REHABILITATION UNIT MANY OF WHOM ARE RESIDENTS OF NEARBY COUNTIES. THE NURSES WERE ALSO EDUCATED ON THE MANAGEMENT OF THE PATIENT USING A LIFE VEST. THIS DEVICE IS AN EXTERNAL DEFIBRILLATOR THAT PROTECTS PATIENTS AT RISK OF SUDDEN CARDIAC DEATH. THE DEVICE IS UTILIZED AS A TRANSITION THERAPY FOR PATIENTS UNABLE TO UNDERGO SURGICAL INTERVENTION FOR THE IMPLANTED DEFIBRILLATOR. WE ALSO ADDED MILRINONE DRIP TO OUR LIST OF SERVICES BEING OFFERED TO THE COMMUNITY. THIS MEDICATION IS ADMINISTERED TO PATIENTS AS A CONTINUOUS INFUSION USING A CALIBRATED INFUSION DEVICE. THE TREATMENT IS INDICATED FOR PATIENTS WITH CONGESTIVE HEART FAILURE. OUR NURSES WERE TRAINED ON PATIENT MONITORING AND INFUSION DEVICE MANAGEMENT. SOME OF THE PATIENTS ON THIS THERAPY HAVE BEEN ABLE TO RETURN TO HOME AFTER REHABILITATING IN OUR SWING UNIT. IN 2019, SAFETY CONTINUED TO BE A PARAMOUNT IN OUR FACILITY. WE TRACKED AND REPORTED COMPLIANCE WITH MEDICATION SCANNING. THIS PRACTICE ENHANCES SAFE MEDICATION ADMINISTRATION BY HELPING VERIFY PATIENT IDENTIFICATION AND CORRECT MEDICATION. WE ALSO ENGAGED IN PRACTICES THAT SUPPORT A SAFE ENVIRONMENT FOR OUR STAFF AS WELL AS OUR PATIENTS. PATIENT MOBILITY IS A SUBSTANTIAL COMPONENT OF REHABILITATION. THE SAFE PATIENT HANDLING COMMITTEE, WHICH IS A MULTIDISCIPLINARY TEAM COMPRISED OF MEMBERS OF OUR NURSING, REHABILITATION, RADIOLOGY, QUALITY, AND FACILITIES STAFF FOCUSED ON THIS INITIATIVE TO ENSURE THAT OUR STAFF ARE INJURY FREE. THE COMMITTEE HAS MADE A TREMENDOUS EFFORT TO DELIVER GUIDANCE AND TRAINING STAFF ON TRANSFERS, BODY MECHANICS, AND USE OF ASSISTIVE DEVICES. THEY HAVE DONE THIS VERY EFFECTIVELY BY INCORPORATING EVIDENCE-BASED PRACTICES IN OUR EVERYDAY TASKS AND ROUTINES, FOR EXAMPLE, IN THE FALL OF 2019 OUR INPATIENT UNIT ADOPTED THE BMAT MODEL (BED SIDE MOBILITY ASSESSMENT TOOL). THIS TOOL ASSISTS WITH ASSESSING PATIENT'S CAPACITY TO MOVE AND COORDINATION STATUS MAKING TRANSFERS SAFER. THE SAFE PATIENT HANDLING COMMITTEE CONTINUES TO MEET REGULARLY TO DISCUSS EDUCATIONAL NEEDS AND PROVIDE HANDS-ON TRAINING TO ALL CLINICAL STAFF. STAFF EDUCATION IS ALSO AN ESSENTIAL ELEMENT IN MAINTAINING AND PROMOTING CLINICAL COMPETENCY. ONE HUNDRED PERCENT OF OUR REGISTERED NURSES MAINTAIN CERTIFICATION IN BASIC AND ADVANCED LIFE SUPPORT THROUGH THE AMERICAN HEART ASSOCIATION AND TELEMETRY. OUR NURSE EDUCATOR PROVIDES EDUCATION OPPORTUNITIES VIA IN-SERVICES, WEBINARS, LECTURES, AND SCHOLARLY ARTICLES. THE HANDS ON TRAINING AND ACADEMIC OFFERINGS ENSURE OUR CLINICAL STAFF IS KNOWLEDGEABLE ON CURRENT TRENDS AND STANDARDS OF CARE. WE LINK ALL NEW HIRES WITH AN EXPERIENCED PRECEPTOR AND ADJUST THE ORIENTATION PROCESS TO MEET THE NEEDS OF EACH INDIVIDUAL. IN OUR FACILITY, EDUCATION DELIVERY INVOLVES EVERYONE. THE NURSE EDUCATOR COLLABORATES WITH THE NURSING LEADERSHIP TEAM, DIRECTOR OF PHARMACY, DIRECTOR OF REHABILITATION SERVICES, CLINICAL IT APPLICATIONS SPECIALIST, AND THE MANAGER OF RESPIRATORY SERVICES TO OFFER COMPREHENSIVE ANNUAL EDUCATION FOR ALL NURSING STAFF.

EMERGENCY DEPARTMENT:ELLENVILLE REGIONAL HOSPITAL EMERGENCY DEPARTMENT IS A VITAL SERVICE FOR OUR COMMUNITY. A RURAL SETTING CAN MAKE IT DIFFICULT FOR RESIDENTS TO ACCESS HIGH-QUALITY EMERGENCY CARE. BY COMBINING QUALITY CARE, EVIDENCE-BASED RESEARCH, EXCELLENT NURSING SKILLS, AND EXPANDING OUR CLINICAL EDUCATION PROGRAMS, THE EMERGENCY DEPARTMENT AT ELLENVILLE REGIONAL HOSPITAL MADE IT OUR GOAL TO IMPROVE PATIENT OUTCOMES ACROSS THE CARE SPECTRUM. (SEE CONTINUATION ON SCHEDULE O)IN 2019, 11,259 PATIENTS RECEIVED TREATMENT IN THE EMERGENCY DEPARTMENT AT ELLENVILLE REGIONAL. WE CONTINUE TO HAVE CONSISTENTLY PROVIDED EXCELLENT CARE IN A MUCH MORE EXPEDIENT MANNER THAN OTHER EMERGENCY DEPARTMENTS ACROSS THE COUNTRY. OUR 2019 AVERAGE FOR ARRIVAL TO TRIAGE IS AT A STELLAR SEVEN MINUTES. ADDITIONALLY, THE AVERAGE LENGTH OF STAY IS AT 107 MINUTES. THESE REMARKABLE TURNAROUND TIMES HAVE BEEN POSSIBLE THANKS TO THE DEDICATION OF OUR MULTIDISCIPLINARY TEAM COMPRISED OF ED PROVIDERS, NURSES, PATIENT CARE TECHNOLOGISTS, REGISTRATION, LAB AND RADIOLOGY. THE TEAM'S PRIMARY FOCUS IS OPTIMAL PATIENT CARE. OUR PATIENTS' NEEDS AND OUR COMMUNITY HAVE ALWAYS BEEN OUR TOP PRIORITY. IMPROVING PATIENT OUTCOMES IS THE PRIMARY DRIVER IN OUR EVERYDAY OPERATIONS. IN 2019, ERH RECEIVED A SILVER AWARD FROM THE AMERICAN HEART ASSOCIATION FOR OUR PARTICIPATION IN THE LIFE LINE PROGRAM. THIS DISTINGUISHED RECOGNITION REPRESENTS OUR COMMITMENT TO GENERATING POSITIVE PATIENT OUTCOMES FOR THOSE LIVING WITH CORONARY ARTERY DISEASE. WE ALSO REMAINED FOCUSED ON THE OPIOID CRISIS. UNARGUABLY WITH OUR PARTICIPATION IN THE MAX (MEDICAID ACCELERATED EXCHANGE) SERIES PROGRAM, WE LEAD THE WAY IN THE WAR AGAINST EXCESSIVE USE OF OPIOIDS IN THE EMERGENCY DEPARTMENT. WE ALSO DEVELOPED PROJECT RESCUE. IN THIS INITIATIVE, OUR ED PROVIDERS OBTAINED CERTIFICATION TO PROVIDE MAT (MEDICATION ASSISTED TREATMENT). THIS INITIATIVE FACILITATED TREATMENT OF PATIENTS PRESENTING TO THE ED AFTER AN OVERDOSE. IN ADDITION, WE COLLABORATED WITH THE SHERIFF DEPARTMENT IN THE ORACLE PROGRAM. THE PRIMARY GOAL OF THE PROJECT IS TO HELP INDIVIDUALS STRUGGLING WITH SUBSTANCE DISORDER BY CONNECTING THEM TO SERVICES. IN 2019, ERH COLLABORATED WITH OUR LOCAL EMS PROFESSIONALS INCLUDING OFFERING CONTINUING MEDICAL EDUCATION ACTIVITIES TO HELP MEET THE NEEDS FOR RELEVANT EDUCATION TO MAINTAIN THE SKILLS TO FUNCTION IN THE FIELD.

RADIOLOGY:IN 2019, RADIOLOGY CONTINUED TO GROW PATIENT EXAM VOLUMES AS WELL AS EXPANDING ON SERVICES BEING PROVIDED TO THE SURROUNDING COMMUNITY. EXAM VOLUMES GREW BY APPROXIMATELY 8% OVER THE PREVIOUS YEAR. ERH ALSO ACQUIRED A NEW RADIOLOGIST GROUP, HUDSON VALLEY RADIOLOGISTS. WITH THE NEW GROUP, WE STARTED OFFERING ULTRASOUND GUIDED BREAST BIOPSIES SO PATIENTS NO LONGER HAVE TO TRAVEL GREAT DISTANCES TO RECEIVE SUCH CRUCIAL HEALTHCARE. THE NEW GROUP IS ON SITE READING TWO DAYS A WEEK AS OPPOSED TO ONLY ONE DAY A WEEK WITH OUR PREVIOUS RADIOLOGIST GROUP. (SEE CONTINUATION ON SCHEDULE O)THE INCREASED ONSITE RADIOLOGIST SCHEDULE HAS ALLOWED US TO EXPAND OUR SCHEDULING OF DIAGNOSTIC MAMMOGRAPHY EXAMS AND IT ELIMINATES ANY DELAY IN PATIENT CARE AS IT ALLOWS FOR THE IMMEDIATE CONSULTATION WITH THE RADIOLOGIST FOR BOTH THE TECHNOLOGIST AND PATIENT. IF ADDITIONAL MAMMOGRAPHY IMAGES OR ULTRASOUND BREAST IMAGING IS NEEDED FOR FURTHER DIAGNOSES, WE NOW CAN PROVIDE SAME DAY IMAGING SO PATIENTS ARE NO LONGER WAITING FOR INITIAL IMAGING RESULTS AND RESCHEDULING THE ADDITIONAL IMAGING.IN CT, WE BECAME ACR ACCREDITED AND CONTINUED TO EXPAND SERVICES PROVIDED SUCH AS CT CALCIUM SCORING. AS A CRITICAL ACCESS HOSPITAL, WE WERE AWARDED A QUALITY IMPROVEMENT GRANT TO SCREEN PATIENTS FOR CARDIOVASCULAR DISEASE AT NO COST TO THEM. THIS SERVICE IS OFFERED TO ALL, INCLUDING PATIENTS WHO DO NOT HAVE HEALTHCARE INSURANCE COVERAGE. WITH THIS GRANT, WE ALSO PROVIDE CAROTID SCREENINGS. BOTH THE CT CALCIUM SCORING AND CAROTID SCREENINGS ARE INTERPRETED BY ONE OF OUR CARDIOLOGISTS AND RESULTS ARE SENT BACK TO THE PATIENT'S PRIMARY CARE PHYSICIAN. THE GRANT HAS ALLOWED US TO COORDINATE, EXPAND ACCESS, CONTAIN COSTS AND IMPROVE THE QUALITY OF CARE TO RESIDENTS IN RURAL AREAS.IN ULTRASOUND, WE PURCHASED A NEW SIEMENS ACUSON SEQUOIA SYSTEM IMPROVING ON THE QUALITY OF CARE PROVIDED. WE ALSO STARTED USING AN ULTRASOUND ENHANCING AGENT TO IMAGE FOCAL LIVER LESIONS. THE COST IS SIGNIFICANTLY LESS THAN HAVING A CT OR MRI AND PROVIDES AN OPTION FOR PATIENTS WITH DIMINISHED KIDNEY FUNCTION AS THE CT/MRI CONTRAST IS LESS TOLERATED BY THESE PATIENTS.
