Organizations Filed Purposes: THE PRIMARY EXEMPT PURPOSE OF THE ORGANIZATION IS TO ORGANIZE, ESTABLISH AND OPERATE: EDUCATIONAL AND VOCATION REHABILITATION FACILITIES, DAY TRAINING, DAY TREATMENT AND DAY HABILITATION PROGRAMS, SERVICE COORDINATION, RESIDENTIAL PROGRAMS, AND RESPITE SERVICES FOR PERSONS WITH CEREBRAL PALSY, DEVELOPMENT DISABILITIES AND OTHER DISABILITIES.

The mission of UCP of Long Island is to advance the independence, productivity and full citizenship of people with cerebral palsy and other disabilities.

UCP's Residential Program maintains Intermediate Care Facilities ("ICFs"), Individual Residential Alternatives ("IRAs") and Residential Habilitation services. The Residential Program serves children and adults with a range of disabilities from moderate to severe. Services are provided by a staff consisting of management, nurses, clinicians, direct support professionals and clerical personnel. The staff is dedicated to furthering the growth and development of the residents in the areas of skills for daily living, socialization, education, recreation and required therapies. Through staff support, the residents are able to increase their independence, sense of freedom, overall quality of life and familiarity with the community. As of December 31, 2019, UCP maintained eleven ICFs and twenty IRAs, all of which are located throughout Suffolk County.

UCP's Education Programs advance the physical, cognitive and emotional growth of children with disabilities, ages birth to 21 years, with year-round comprehensive programs, screenings, evaluations, therapies, home/center-based instruction, family training and support groups and assisted technology services. The Early Intervention Program is designated for children from birth to two years and their families. This program provides infant and early childhood services to maximize strengths and minimize weaknesses in a child's overall development. Services are available in the home as well as at UCP's center. The Pre-School Program is for children from three to five years old, and the School Age Program is for young people age five to twenty-one, with multiple disabilities. Both programs are funded by The New York State Education Department. UCP operates two classrooms for the Pre-School Program and seven for the School Age Program. These classes are located at UCP's education center.

The Day Treatment program strives to increase the independence of individuals with disabilities who are over 21 years of age (including senior citizens) by providing life enrichment programs and preparatory services for possible work activities. The program serves approximately 90 adults, ambulatory and non-ambulatory, with developmental disabilities such as autism, epilepsy, mental retardation, neurological symptoms and cerebral palsy. Form 990, Part III, Line 4D In addition to UCP's three largest program activities, it also provides the following services to developmentally disabled individuals: day habilitation, respite programs, service coordination and community habilitation. The Day Habilitation Program assists individuals with development disabilities to acquire independent living skills through activities that are both facility and community based. UCP relocated the program Bohemia to the Hauppauge center in 2019. The Hauppauge center is certified for 123 individuals. Service Coordination uses a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan for the people receiving services. During 2018, the program transitioned to a Care Coordination model, which is the first phase of New York State implementing managed care for persons with developmental disabilities. In June 2019, all UCP employees working in the Service Coordination program were transitioned to the Care Coordination agency and UCP no longer provides Service Coordination services. Respite services at UCP provide relief to individuals who are responsible for the primary care and support of an individual with a developmental disability. UCP provides respite services both in and out of the home and operates programs seven days a week during the day, evenings and overnight hours. UCPs Community Habilitation program is a New York State Office for Persons with Developmental Disabilities ("OPWDD") waiver service that consists of services designed to assist individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to reside successfully in home and community-based settings. UCP's Employment Services provide evaluation, training and job placement in competitive or supported employment. The program operated a sheltered work activity area for individuals with disabilities, which ceased operations in January 2018. The population includes individuals with physical disabilities, developmental disabilities, learning disabilities and traumatic head injuries.
