The Aging in New York Fund is dedicated to enhancing the quality of life for older New Yorkers and their families.
Successful Aging for Increased Longevity (SAIL) Program:The Aging in New York Fund has partnered with Flushing Hospital and Chinese-speaking members of senior centers to help reduce social isolation and depression. Originally designed for in-person outreach, staff conducted virtual outreach and using technology to connect with Chinese residents adversely affected by the pandemic. Chinese-speaking seniors, who may be averse to mental health interventions, were helped to overcome fear and stigma associated with mental health issues. The staff connected seniors by phone and other technology to Chinese-speaking therapists at Flushing Hospital clinics for counseling and medications where necessary.
CCL - Community Care Link:This program is a new model management services organization, implemented by DFTA and the Aging in New York Fund, in collaboration with Healthfirst. The goal is to reach out to Healthfirst members to help them connect with a primary care provider, based on lists provided by Healthfirst. The program is a collaborative of 5 other CBOs JASA, Selfhelp, SAGE, Bay Ridge Senior Center, and Carter Burden. The members, English and Spanish-speakers, are Medicare recipients, identified for each agency by zip code, who havent had contact with a primary care physician during 2019, based on Healthfirst records. The program was conceived before the pandemic. It was revised to better meet client needs as people self-quarantined and were forced to remain in their homes.Each agency is reimbursed at set rates for the services they provide to clients, using Peer Place and STARS to document each call.
Relatives as Parents Program (RAPP)and other program services:The primary focus of the RAPP program was originally an in-person support groups and intergenerational program, but due to the pandemic a major adjustment was implemented to pivot the program and conduct all its activities in a virtual format. The challenge of virtual engagement was reduced in part through the provision of iPads to NYCHA residents. Support group discussions and intergenerational programs were modified to address pressing issues such as social justice and inequity and the Black Lives Matter movement. Support groups dealt with the new challenges and stresses of virtual classrooms, social distancing and lack of social interactions with friends and family.Other programs included Assigned Housing Counsel, Grand Parent Resource Center Programs etc.
