Support Services include short term mental health multilingual care management services for individuals with chronic diseases including HIV/AIDS, diabetes, substance abuse, and psychiatric illnesses including Health Home Care Management Services, care management for patients on PrEP/PEP, and care management for transgender patients care management for patients with Hepatitis B and C
