TO PARTICIPATE IN, DEVELOP PROGRAMS AND/OR FACILITATE THE PARTICIPATION IN PROGRAMS FUNDED AND/OR OVERSEEN BY GOVERNMENTAL AGENCIES, AND/OR SPONSORED THROUGH FOUNDATION(S) AND/OR GRANT OR OTHER FUNDING, IN EACH CASE, THE PURPOSE OF WHICH IS THE IMPROVEMENT OF THE OVERALL QUALITY OF LIFE FOR INDIVIDUALS IN THE STATE OF NEW YORK, PRIMARILY INDIGENT CHILDREN AND HIGH COST/HIGH NEED MEDICAID ENROLLEES WITH MEDICAL, BEHAVIORAL AND/OR DEVELOPMENTAL DISABILITIES IN UNDERSERVED AREAS.
FOUNDED IN EARLY 2014, CCF WAS FORMED IN ANTICIPATION OF CHANGES IN THE DELIVERY OF HEALTH CARE SERVICES TO CHILDREN AND FAMILIES ELIGIBLE FOR NEW YORK STATE'S MEDICAID PROGRAM. CCF IS A NEW YORK STATE NONPROFIT COMPRISED OF SUB-CONTRACTED CARE MANAGEMENT AGENCIES THAT SERVE CHILDREN AND FAMILIES THROUGHOUT THE FIVE BOROUGHS OF NEW YORK CITY AND WESTCHESTER, NASSAU, AND SUFFOLK COUNTIES. CCF OFFERS COMPREHENSIVE SERVICES TO CHILDREN AND FAMILIES IN A MORE COORDINATED MANNER THAN COULD BE ACHIEVED INDIVIDUALLY.
INTEGRATION AND COORDINATION OF CARE IN THE PROVISION OF BEHAVIORAL AND MENTAL WELLNESS SERVICES. IN ADDITION, INFORMATION WAS DISSEMINATED TO THE PUBLIC THAT SUPPORTS EDUCATION, INCLUDING BUT NOT LIMITED TO PROVIDING LECTURES AND COUNSELING SERVICES ABOUT THE NEEDS OF INDIVIDUALS WITH MEDICAL, BEHAVIORAL AND LONG TERM CARE NEEDS.THE CCF HEALTH HOME ENTERED INTO A CONTRACT THROUGH 2019 TO BECOME A PARTICIPATING PARTNER FOR THE MT. SINAI HOSPITAL SYSTEM'S PERFORMING PROVIDER SYSTEM. CCF PROVIDES MT. SINAI WITH DATA AND OUTCOMES UNDER THE UMBRELLA OF THE STATE'S DELIVERY SYSTEM REFORM INCENTIVE PROGRAM (DSRIP) WHICH WAS CREATED BY THE MEDICAID REDESIGN TEAM UNDER GOVERNOR CUOMO TO ADDRESS MEDICAID SPENDING AND BETTER HEALTH OUTCOMES FOR THEIR MEDICAID RECIPIENTS. THE DATA SHARED BY CCF IS SPECIFIC TO MEASURING THE LEVEL OF AWARENESS AND EDUCATION/TRAINING THAT CCF IS PROVIDING TO THE COMMUNITY AT LARGE AND MEDICAL PROVIDERS ABOUT THE VALUE OF HEALTH HOME CARE COORDINATION TO INCREASE AWARENESS AND REFERRALS OF MEMBERS.
THE NY STATE DEPARTMENT OF HEALTH AWARDED A CONTRACT TO CHAPIN HALL AT THE UNIVERSITY OF CHICAGO TO DEVELOP AND OPERATE A TECHNICAL ASSISTANT INSTITUTE TO ADDRESS THE NEEDS AND EDUCATION OF HEALTH HOME CARE MANAGERS WHO ADMITTED THE CANS-NY ASSESSMENT TO MEASURE ACUITY LEVELS FOR CHILDREN ENROLLED IN THE HEALTH HOME SERVING CHILDREN PROGRAM. CCF IS SUB CONTRACTED TO CHAPIN HALL SERVING THE NEEDS OF THE DOWNSTATE REGION OF NY STATE. UNDER THIS CONTRACT, CCF WILL PROVIDE TRAINING AND TECHNICAL ASSISTANCE TO ALL USERS OF THE CANS-NY ASSESSMENT TOOL.
