THE FUND FOR PUBLIC HEALTH IN NEW YORK, INC. (FPHNY) IS DEDICATED TO THE ADVANCEMENT OF THE HEALTH AND WELL-BEING OF ALL NEW YORK CITY RESIDENTS. TOWARDS THIS AIM, THE FUND IMPLEMENTS PROGRAMS TO ADDRESS PRESSING PUBLIC HEALTH NEEDS, DEVELOPS PRIVATE SECTOR SUPPORT FOR ENHANCING THE HEALTH OF NEW YORKERS, AND EDUCATES NEW YORKERS ABOUT THE ROLE THEY CAN PLAY IN PROTECTING THEIR OWN HEALTH AND THE HEALTH OF THEIR FAMILIES AND COMMUNITIES.
FPHNY ADDRESSES PRESSING PUBLIC HEALTH NEEDS, FOSTERS PRIVATE SECTOR SUPPORT TO ENHANCE HEALTH AND HEALTHCARE, AND HELPS TO EDUCATE THE PUBLIC REGARDING THE PROTECTION OF INDIVIDUAL, FAMILY AND COMMUNITY HEALTH.
PURSUANT TO A NEGOTIATED ACQUISITION AGREEMENT, FPHNY PROVIDES SERVICES TO DOHMH ON A REQUIREMENTS BASIS TO DEVELOP AND IMPLEMENT COMMUNITY BASED PUBLIC HEALTH PROJECTS. TWO EXAMPLES OF PROJECTS INCLUDE: HARLEM HEALTH ADVOCACY PROJECT - TO REDUCE DISPARITIES IN CHRONIC DISEASE AMONG PUBLIC HOUSING RESIDENTS AND TO IMPROVE THEIR LONG-TERM HEALTH AND QUALITY-OF-LIFE BY TRAINING RESIDENTS TO SERVE AS COMMUNITY HEALTH WORKERS AND HEALTH ADVOCATES; AND THE STELLAR FARMERS MARKETS PROGRAM WHICH PROVIDES FREE, BILINGUAL NUTRITION EDUCATION AND COOKING DEMONSTRATIONS AT SELECT FARMERS MARKETS THROUGHOUT NYC.
THE FEDERAL FINANCIAL PARTICIPATION (FFP) 90/10 (EP2) GRANT PROVIDED BY THE NEW YORK STATE DEPARTMENT OF HEALTH (NYSDOH), IN CONNECTION WITH THE ELECTRONIC HEALTH RECORDS PROJECT, IS A CORE AND MILESTONE-BASED GRANT. ULTIMATELY, IT WAS FUNDED VIA PASS-THROUGH FUNDS PROVIDED TO NEW YORK STATE FROM THE FEDERAL GOVERNMENT (DHHS). THE GRANT PROVIDES CONTINUATION SERVICES FOR PROVIDERS NOT COVERED BY NYC REACH, I.E. OUT OF SCOPE PROVIDERS DEFINED AS PRIMARY CARE PROVIDERS AT LARGE PRACTICES WHO DO NOT MEET THE ELIGIBILITY REQUIREMENTS DEFINED BY THE DEPARTMENT OF HEALTH AND HUMAN SERVICES OFFICE OF THE NATIONAL COORDINATOR FOR HEALTH INFORMATION TECHNOLOGY (HHS ONC), AS WELL AS SPECIALISTS THAT ARE ELIGIBLE FOR MEDICAID MEANINGFUL USE INCENTIVES. PRIMARY CARE PROVIDERS ARE OUT OF SCOPE BASED UPON SITE CAP LIMITATIONS DETERMINED BY HHS ONC.ACTIVITIES INCLUDING CORE WORK IS DONE WHICH SERVES THE ENTIRE POPULATION SERVED IN THE AREAS OF (1) OUTREACH AND RECRUITMENT AND (2) EDUCATION. OUTREACH AND RECRUITMENT ARE DEFINED AS THE IDENTIFICATION AND RECRUITMENT OF ELIGIBLE PROVIDERS THROUGH EXISTING COMMUNITY HEALTH INFORMATION TECHNOLOGY NETWORKS AND OTHER MEMBERSHIP AND ADVOCACY ORGANIZATIONS, SUCH AS HOSTING MINI-CONFERENCES AND WEBINARS, AND LEVERAGING SOCIAL MEDIA TO COMMUNICATE TO SPECIFIC STAKEHOLDER GROUPS. EDUCATION ARE PROGRAMS DELIVERED EITHER ONSITE, OR BY WEBINAR WHICH FOCUS ON PROVIDER ADOPTION OF EHR TECHNOLOGY, HEALTH INFORMATION EXCHANGE AND ADVANCEMENT OF CARE COORDINATION THROUGH THE USE OF TECHNOLOGY, SUCH AS CONDUCTING TRAINING ON MEANINGFUL USE (MU) ELIGIBILITY, APPLICATION, ATTESTATION PROCESSES AND TOOLS AND MATERIALS ON PRIVACY AND SECURITY, AND PATIENT CENTERED MEDICAL HOMES.IN ADDITION, THE PROGRAM INCLUDES TWO DIFFERENT MILESTONES (1) IMPLEMENTATION OF EHR AND (2) ATTESTATION TO ADOPTION, IMPLEMENTATION AND UPGRADE (AIU) OF CERTIFIED EHR. FOR IMPLEMENTATION OF EHR, STAFF PROVIDES ASSISTANCE TO THE PROVIDERS IN THE IMPLEMENTATION OF THE NEW TECHNOLOGIES. IMPLEMENTATION SUPPORT INCLUDES (BUT IS NOT LIMITED TO), DETERMINATION OF READINESS, ASSISTANCE IN VENDOR SELECTION, PROJECT MANAGEMENT, AND DESIGN OF WORKFLOW TO BE ADAPTED FOR THESE NEW TECHNOLOGIES. ATTESTATION TO ADOPTION, IMPLEMENTATION AND UPGRADE (AIU) OF CERTIFIED EHR ACTIVITIES INCLUDES GUIDANCE ON HIPPA, ASSISTANCE IN ACHIEVEMENT OF AND SUCCESSFUL ATTESTATION TO MU STAGE 1 AND CLINICAL QUALITY MEASURES.
MANY RISK FACTORS FOR CARDIOVASCULAR DISEASE (CVD) AND DIABETES MELLITUS (DM), INCLUDING OBESITY, HIGH BLOOD PRESSURE (HBP), AND HIGH BLOOD CHOLESTEROL (HBC), DISPROPORTIONATELY AFFECT NEW YORK CITY'S (NYC) LOW-INCOME RESIDENTS AND PEOPLE OF COLOR. DISPARITIES BY GEOGRAPHY ARE ALSO STARK, OFFERING COMPELLING EVIDENCE FOR CONCENTRATING RESOURCES AND SERVICES IN AREAS OF HIGHEST NEED. THE TAKE ACTION ABCD PROJECT IS AN INITIATIVE TO IMPROVE A1C CONTROL, BLOOD PRESSURE CONTROL, CHOLESTEROL MANAGEMENT AND DIABETES PREVENTION FOR ADULT RESIDENTS OF NYC. THROUGH A SUITE OF COMPLEMENTARY STRATEGIES, TAKE ACTION AIMS TO IMPACT A LARGE NUMBER OF ADULTS ACROSS THE CITY IN 24 NEIGHBORHOODS WHERE DM AND CVD IS HIGHER THAN CITY AVERAGES AND TO SUPPLEMENT ACTIVITIES IN A SUBSET OF EIGHT NEIGHBORHOODS TO REDUCE BARRIERS AND DISPARITIES IN HEALTH FOR A PRIORITY POPULATION UNDULY AFFECTED BY DM AND CVD.FOR LARGE POPULATION IMPACT, DOHMH PARTNERS WITH HEALTH CARE SITES AND INDEPENDENTLY OWNED PHARMACIES TO PROMOTE PROACTIVE HEALTH CARE TEAMS WHERE PROVIDERS HAVE INCREASED KNOWLEDGE, SKILLS AND TOOLS TO INCORPORATE TRACKING AND MONITORING OF CLINICAL MEASURES; PRACTICES ADOPT TEAM-BASED CARE, WITH NON-PHYSICIAN AND PHYSICIAN TEAM MEMBERS COORDINATING AND COLLABORATING TO DELIVER PATIENT-CENTERED CARE; MORE PROVIDERS USE ELECTRONIC INFORMATION SYSTEMS AND SHARE INFORMATION AMONG TEAM MEMBERS, PATIENTS, OUTSIDE PROVIDERS, AND OTHER SETTINGS; MORE PROVIDERS USE EVIDENCE-BASED TOOLS AND QUALITY IMPROVEMENT APPROACHES FOR PREVENTION, ASSESSMENT AND MANAGEMENT; AND HEALTH CARE IS COORDINATED ACROSS THE CONTINUUM OF CARE ACROSS CLINICAL AND COMMUNITY SETTINGS.
