SERVICES INCLUDE SUPPORTIVE COUNSELING, GROUPS, CASE MANAGEMENT, ENTITLEMENTS SUCH AS FOOD STAMPS, HEALTH INSURANCE, COORDINATING HOME CARE AFTER HOSPITALIZATIONS, ARRANGING HOME CARE AND HOUSEKEEPING SERVICES.
