Organizations Filed Purposes:
The plan provides life insurance, hospital, major medical, vision, prescription drug, vacation and legal benefits for eligible participants. The effective date of coverage for all benefits is the first day of the month that at least thirty shifts have been completed during two consecutive months. Benefits coverage will cease at the end of the calendar month following the calendar month during which a participant terminates covered employment.
THE PLAN PROVIDES HOSPITAL, MAJOR MEDICAL, LIFE INSURANCE, VISION, PRESCRIPTION DRUG AND VACATION BENEFITS FOR ELIGIBLE PARTICIPANTS.
