AAYHC provides two types of services: Home and Community Support Services (HCSS) through the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid Waivers, and Personal Care Aide (PCA) service through Medicaid Managed Care or Managed Long Term Care.(continued on Schedule O)HCSS are utilized when oversight and/or supervision as a discrete service is necessary to maintain the health and welfare of the participant living in the community. Oversight and/or supervision may be needed for safety monitoring to prevent an individual from harmful activities (for example wandering or leaving the stove on unattended). Oversight and/or supervision can be accomplished through cueing, prompting, direction and instruction. If the applicant/participant does not require oversight and/or supervision, HCSS would not be appropriate. HCSS can also be provided to participants needing oversight and/or supervision who also require assistance with personal care services. Since HCSS staff must be trained at the Personal Care Aide (PCA) level, they are able to perform the scope of personal care tasks and functions necessary for an individual who also requires oversight and/or supervision. The HCSS worker is never allowed to exceed this scope of practice. HCSS is complementary but not duplicative of other services. HCSS is not to be used as a companion service. Individuals can only be approved for HCSS through the NHTD or TBI waiver.Personal Care Aides provide the participant one-on-one supervision for safety and assistance with activities of daily living to maintain the participants independence in their home. PCA services can be approved through Medicaid Managed Care, Managed Long Term Care, or can be privately paid by the individual.Both HCSS and PCA staff must complete the New York State Department of Health approved PCA training in order to be eligible to provide these services. Individuals are eligible for services for the TBI waiver if they:1. have a traumatic brain injury 2. are a Medicaid recipient3. are between the ages of 18 and 64 while applying to the waiver4. determined to need a nursing home level of care as a result of their TBI5. choose to participate in the waiver instead of a nursing home6. complete an Initial Service Plan with their Service Coordinator and it is approved by the Regional Resource Development Specialist along with a Plan for Protective OversightIndividuals are eligible for services for the NHTD waiver if they:1. must be eligible for admission to a nursing home2. have Medicaid coverage for Community Based Long Term Services3. be at least 18 years of age or older (must have a physical disability for those 18-64)4. be able to live in the community with assistance from family, friends, community members, etc., non Medicaid supports and/or Medicaid state plan and in need of one or more waiver services per month5. be a part of the group of NHTD waiver participants whose average cost of Medicaid services is less than what it would cost for the same group to live in a nursing home6. choose to live in the community as a participant in this waiver rather than in a nursing home; and7. not participate in another Home and Community Based Services waiver.
