His Branches Health - His Branches is uniquely neighborhood-centric, with two practice locations in Rochester New York: Grace Family Medicine in the 19th Ward, and Joy Family Medicine in the Beechwood neighborhood. In our 2019/2020 fiscal year we served 1,947 patients, of all ages, through 7,123 patient encounters. In caring for the whole person, we address all aspects of well-being: body, mind, and spirit. We specialize in meeting complex needs and removing barriers to good health. We offer group learning in chronic conditions, on-site lab services, care management, and case management to facilitate the connection to much needed social support services.This program is supported by contributions.
His Branches Wellness - His Branches is rooted in the communities it serves, striving to enrich the health and well-being not only of our patients, but the communities where they work and live. Programs and services include a pregnancy resource center that delivers a wide range of education programs to both women and men in parenting and life skills, sexual risk avoidance, abortion recovery, and sexual healing; and a program called Healthy Me!, a weekly interactive group learning session in healthy living including nutrition, movement, spiritual care, and chronic disease management.This program is supported by contributions.
His Branches Community - In the neighborhood, we coordinate an annual neighborhood summer block party, a bike clinic, a neighborhood garden, and activities with the neighborhood library and schools. We partner with other community based organizations throughout the year to create special festive events like Christmas dinners or boutique-like shopping for winter coats or summer dresses so neighbors feel they are receiving something special, not just material aid.This program is supported by contributions.
