NYC DEPARTMENT FOR THE AGING TITLE IIIB: PROVIDING COMPREHENSIVE, COMMUNITY SOCIAL SERVICES TO OLDER ADULTS, AGE 60 YEARS AND OLDER, IN THE FORMS OF CASE ASSISTANCE, INCLUDING INFORMATION AND ASSISTANCE WITH ENTITLEMENTS AND BENEFITS APPLICATIONS; CASE MANAGEMENT FOR HOME-BOUND OLDER ADULTS, INCLUDING THOSE RECEIVING HOME DELIVERED MEALS AND NYS EISEP HOME CARE; ELDER ABUSE PREVENTION AND INTERVENTION SERVICES FOR VICTIMS OF ELDER ABUSE, INCLUDING SAFETY PLANNING, EDUCATION, COUNSELING, ADVOCACY, COURT ACCOMPANIMENT, LOCK REPLACEMENT AS WELL AS COMMUNITY EDUCATION; TRANSPORTATION FOR FOOD SHOPPING, TO/FROM SENIOR CENTERS, RECREATIONAL TRIPS AND LIMITED MEDICAL APPOINTMENTS; 2 NEIGHBORHOOD SENIOR CENTERS, PROVIDING 2 HOT MEALS DAILY, SOCIALIZATION, CASE ASSISTANCE, EDUCATIONAL, RECREATIONAL, ARTS AND CULTURE AND WELLNESS PROGRAMS; NY CONNECTS, PROVIDING INFORMATION AND REFERRAL FOR LONG TERM CARE SERVICES AND SUPPORT NEEDS FOR PEOPLE OF ALL AGES WITH DISABILITIES AND THEIR CAREGIVERS.

NYS OFFICE OF VICTIM SERVICES: PROVIDING SERVICES TO ELDERLY VICTIMS OF CRIME, INCLUDING: NYS OVS CLAIM APPLICATION INFORMATION AND ASSISTANCE TO SUBMIT ELIGIBLE CLAIMS, ADVOCACY, SAFETY PLANNING AND COUNSELING FOR BOTH CRIME VICTIMS AND VICTIMS OF ELDER ABUSE, COURT ACCOMPANIMENT, INCLUDING TRANSPORTATION IF NEEDED, LOCK REPLACEMENT AND SUPPORT GROUPS.
