Organizations Filed Purposes: TO STRENGTHEN FAMILIES IN OUR COMMUNITY THROUGH COUNSELING, EDUCATION, AND SUPPORT. GREENLEAF'S CORE VALUES ARE FAMILY, RESPECT, INTEGRITY, INTERDEPENDENCE, AND EXCELLENCE.

COUNSELING, EDUCATION, & SUPPORT FOR INDIVIDUALS, GROUPS, THE DEAF, AND PREGNANT/PARENTING FAMILIES.

BEHAVIORAL HEALTH (BH) - BH SERVICES PROVIDES COUNSELING BY USING EVIDENCED-BASED BEST PRACTICES (EBP) TO INDIVIDUALS AND FAMILIES STRUGGLING WITH CHALLENGES OF MENTAL ILLNESS, SUBSTANCE USE, POVERTY, DIVERSITY, AND LIFE'S EVERYDAY STRUGGLES. EBP PRACTICES INCLUDE COGNITIVE BEHAVIORAL THERAPY, MOTIVATIONAL INTERVIEWING, EYE MOVEMENT DE-SENSITIZATION AND RE-PROCESSING (EMDR) AND FEEDBACK INFORMED TREATMENT (FIT) FOR OUTCOMES AND CLIENT ENGAGEMENT. THESE SERVICES ARE PROVIDED THROUGH INDIVIDUAL, COUPLES, FAMILY AND GROUP COUNSELING. A BIO-PSYCH-SOCIAL MODEL AND INTEGRATED APPROACH TO TREATING CO-OCCURRING DISORDERS FOR SUBSTANCE ABUSE AND MENTAL ILLNESS ARE UTILIZED FOR INTERVENTIONS. ADDITIONAL PROGRAMS INCLUDE A VARIETY OF PARENTING AND ANGER MANAGEMENT GROUPS. COUNSELING AND CASE MANAGEMENT SERVICES ARE ALSO PROVIDED IN A SCHOOL-BASED PROGRAM AT COVENTRY LOCAL SCHOOLS. WE CONTINUED TO PROVIDE SUMMIT COUNTY COURT OF COMMON PLEAS WITH INTERVENTION IN LIEU OF CONVICTION ASSESSMENTS AND RECOMMENDATIONS. IN 2019 BEHAVIORAL HEALTH CONTINUED TO EVALUATE PROGRAM NEEDS. WE SAW A REDUCTION IN THE NEED FOR INTENSIVE OUTPATIENT SERVICES FOR SUBSTANCE USE DISORDERS, BUT A INCREASED NEED FOR OUTPATIENT SUBSTANCE USE PROGRAMING. WE DEVELOPED A WOMEN SEEKING SAFETY GROUP TO ADDRESS SUBSTANCE USE AND TRAUMA.

COMMUNITY SERVICES FOR AND OF THE DEAF (CSD) - CSD SUPPORTS AND ADVOCATES FOR THE RIGHTS OF DEAF, HARD OF HEARING, DEAFBLIND, AND INDIVIDUALS WITH HEARING LOSS IN SUMMIT, PORTAGE, MEDINA, WAYNE, AND HOLMES COUNTIES. CSD PROVIDES SERVICES AND EDUCATION TO ENHANCE THE QUALITY OF LIFE FOR INDIVIDUALS SERVED BY THE PROGRAM. SERVICES INCLUDE 24/7 AMERICAN SIGN LANGUAGE INTERPRETING, ADVOCACY AND SUPPORT SERVICES, HEARING LOSS RESOURCES, JOB READINESS TRAINING, AMERICAN SIGN LANGUAGE CLASSES, CULTURAL COMPETENCY TRAINING, AND TRAINING MENTAL HEALTH PROFESSIONALS ON DEAF CULTURE. IN 2019, CSD CONTINUED TO PROVIDE OVER 11,000 HOURS OF DIRECT SERVICE TO DEAF, DEAFBLIND, AND HARD OF HEARING COMMUNITY. ADDITIONALLY, CSD EXPANDED SERVICES IN PARTNERSHIP WITH OPPORTUNITIES FOR OHIOANS WITH DISABILITIES TO PROVIDE EMPLOYMENT SERVICES TO THE COMMUNITY SERVED. ONGOING CSD GOALS INCLUDE A GREATER ARRAY OF SERVICES FOR THE HARD OF HEARING POPULATION, INCREASED SOCIAL ACTIVITIES FOR THE DEAF COMMUNITY, INCREASED LITERACY IN THE DEAF COMMUNITY, AND AMERICAN SIGN LANGUAGE FOR DEAF CHILDREN AND THEIR FAMILIES.

MOMS AND BABIES FIRST PROGRAM (MABF) - THE MABF'S MISSION IS TO PROVIDE EDUCATION AND SUPPORT TO AFRICAN AMERICAN PREGNANT WOMEN IN SUMMIT COUNTY. THE PROGRAM AIMS TO REDUCE AND ELIMINATE DISPARITIES IN INFANT MORTALITY AMONG AFRICAN AMERICANS, IMPROVE BIRTH OUTCOMES, INCREASE EARLY PRENATAL CARE AND INCREASE THE OVERALL UTILIZATION OF ROUTINE INFANT HEALTHCARE. EARLY AND CONTINUOUS PERINATAL CARE IS AN IMPORTANT STEP TOWARD ASSURING THAT THE MOTHER AND INFANT WILL BE HEALTHY THROUGHOUT THE PREGNANCY AND DELIVERY. MABF FOLLOWS THE MOTHER AND INFANT UNTIL THE CHILD REACHES ITS FIRST BIRTHDAY. THIS PROGRAM IS A PARTNERSHIP BETWEEN SUMMIT COUNTY PUBLIC HEALTH AND GREENLEAF FAMILY CENTER.CERTIFIED COMMUNITY HEALTH WORKERS (CCHW) ARE AVAILABLE TO ASSIST CLIENTS WITH MAKING PRENATAL CARE APPOINTMENTS, APPLY FOR MEDICAL COVERAGE, CONNECT HOUSING AND WIC (WOMEN, INFANTS AND CHILDREN), LOCATE RESOURCES TO MEET FAMILY'S NEEDS AND DEVELOP AND REACH FAMILY GOALS THROUGH HOME VISITATION. A HOME VISIT IS SCHEDULED IN ADVANCE (NOT A CHANCE MEETING), LASTS 60-90 MINUTES, TAKES PLACE IN THE FAMILY HOME GENERALLY, AND CAN CONSIST OF: - A FACE-TO-FACE INTERACTION BETWEEN THE HOME VISITOR AND A FAMILY OR PARENT - OFFERING SOCIAL SUPPORT AND DISCUSSION OF TOPICS THAT ARE IMPORTANT TO FAMILIES. - PLANNED ACTIVITIES AND TOPICS BASED ON THE FAMILY'S NEEDS AND THE GOAL OF THE PROGRAM. - PLANNING BEFORE THE VISIT AND FOLLOW-UP AFTER THE VISIT.COMMUNITY HEALTH WORKERS CONDUCT MONTHLY HOME VISITS WITH THEIR CLIENTS.MABF LONG TERM GOAL FOR THIS YEAR IS TO INCREASE FROM 49% TO 65% THE PERCENTAGE OF THE MOTHERS THAT BREASTFEED THEIR NEWBORN BABIES.
