Organizations Filed Purposes: For 26 years, AVOC has provided academic enrichment, intervention and prevention services in many communities throughtout Ohio. The mission is to assist in breaking the cycles of illiteracy, poor health, teen dropout, youth imprisonment and poverty.

Presently, A Vision of Change has designed a Public Health Model of building Community Health Hubs utilizing CSU Community Health Workers and CSU student nurses as boots on the ground. With the additional support of the Cuyahoga County Board of Health, Fairhill Partners and the Prevention Research Center for Health Neighborhoods at Case Western Reserve University, the intent is to expand on our organizing skills, outreach/recruitment and promotion/dissemination strategies by building a pipeline of opportunities for residents/CHWs across the spectrum of achieving health equity and promoting health care efforts.

In 2019, AVOC, home of the Greater Cleveland Community Health Workers Association (GCCHWA), increased its capacity to serve as the community engagement catalyst for Cuyahoga Countys community health initiatives housed under the HIP-Cuyahoga umbrella, known as REACH. AVOC not only provides support as the connector of the clinic to community linkages strategy but the core programs of AVOC braided with Asset Based Community Development (ABCD) to work collaboratively with the all functions of the initiative helping to develop and shape a comprehensive community engagement framework

The objectives of the AVOC Youth Development project for youth in the Cuyahoga County juvenile detention center is to introduce youth to techniques that help reduce stress and anxiety, improve attention and focus skills and personal relationships with other people.AVOC is prepared to offer programs that inform youth on how to make healthier choices and raise awareness to the importance of physical activities.In 2019 we implemented our AVOC Youth Development Project and we are looking forward to coming back this year to service the youth at the Juvenile Court Division. The youth who benefited from our programs were both male and females ages 12-20. Over the program period, we had a 100% participation rate, because it was a prepared audience. Of that group, only 1% was non-participation, and an invitation was always extended by a member of AVOC.
