Organizations Filed Purposes: THE COUNCIL OF AGING PROVIDES AGING ADULTS AND THEIR SUPPORT SYSTEM WITH SERVICES AND OPPORTUNITIES THAT ENABLE THEM TO ACHIEVE AN OPTIMAL, SELF DEFINED, QUALITY OF LIFE. THIS IS ACHIEVED THROUGH THE PROVISION OF EDUCATION, NUTRITION SERVICES, CASE MANAGEMENT SERVICES, IN-HOME ASSISTANCE AND COMMUNITY OUTREACH. THE LAKE COUNTY COUNCIL ON AGING PROVIDES SUPPORT SERVICES, PROGRAMS AND ADVOCACY TO HELP LAKE COUNTY SENIORS LIVE DIGNIFIED AND INDEPENDENT LIVES. THE AGENCYS PRIMARY RESPONSIBILITIES ARE TO DESIGN, PROMOTE AND IMPLEMENT SERVICES TO ADDRESS THE IDENTIFIED NEEDS OF THE COMMUNITYS ELDER POPULATION AND TO COORDINATE EXISTING SERVICES IN PARTNERSHIP WITH PUBLIC AND PRIVATE ORGANIZATIONS. THE AGENCY PROVIDES INFORMATION, ASSISTANCE AND DIRECT SUPPORT THAT ALLOW SENIORS TO MAINTAIN THEIR INDEPENDENCE AND SUSTAIN THEIR QUALITY OF LIFE.

MEALS ON WHEELS - OUR COMMUNITY-BASED HOME DELIVERED MEAL PROGRAM (AKA MEALS ON WHEELS) MAKES IT POSSIBLE FOR FRAIL OLDER ADULTS, AGED 60 AND OVER, TO REMAIN INDEPENDENT IN THEIR HOMES BY PROVIDING DAILY DELIVERY OF A NUTRITIOUS MEAL. EACH OF THE MEALS IS REVIEWED BY A REGISTERED DIETICIAN THROUGH THE WESTERN RESERVE AREA AGENCY ON AGING TO ENSURE THAT THEY PROVIDE A MINIMUM OF ONE-THIRD OF THE DAILY RECOMMENDED NUTRITIONAL ALLOWANCES SENIORS NEED FOR GOOD HEALTH. ADDITIONALLY, A BRIEF VISIT FROM THE VOLUNTEERS WHO DELIVER THE MEALS SERVES AS BOTH A FRIENDLY VISIT AND A CHANCE TO CHECK ON THE SENIORS WELL-BEING. IN 2019, 472 VOLUNTEERS DELIVERED 173,746 MEALS TO APPROXIMATELY 1,000 HOMEBOUND, FRAIL AND DISABLED SENIORS MONDAY THRU FRIDAY FROM FIVE LOCATIONS ACROSS LAKE COUNTY, OHIO. ALL MEALS ARE PROVIDED AT NO COST, THOUGH A DONATION IS GRATEFULLY ACCEPTED.

SOCIAL WORK THE COUNCIL ON AGING SOCIAL WORK DEPARTMENT SERVES AS A LINK BETWEEN THE CLIENTS WE SERVE AND THE COMMUNITY RESOURCES THAT MAKE INDEPENDENT LIVING POSSIBLE. LICENSED SOCIAL WORKERS ASSESS, DEVELOP CARE PLANS, IMPLEMENT, MONITOR AND COORDINATE A RANGE OF SERVICES AND SUPPORTS TO MAINTAIN THE INDEPENDENCE, HEALTH AND SAFETY OF FRAIL OLDER ADULTS AND INDIVIDUALS, AGED 60 AND OVER, WHO ARE IN NEED OF ASSISTANCE. IN 2019, THE FOLLOWING SERVICES WERE CONDUCTED BY LICENSED PROFESSIONALS TO DEVELOP PLANS OF ASSISTANCE THROUGHOUT LAKE COUNTY, OHIO: PROVIDED SERVICES TO 1,335 UNDUPLICATED SENIORS, PROVIDED 2,280 IN-HOME VISITS (A 5% INCREASE FROM 2018), COMPLETED 598 NEW CLIENT PSYCHOSOCIAL ASSESSMENTS, 449 ANNUAL REASSESSMENTS, 3,787 DIRECT SERVICE HOURS, AND 9,490 TOTAL UNITS OF SERVICE. ADDITIONALLY, SENIORS ARE PROVIDED VIALS-OF-LIFE WHICH PROVIDE EMERGENCY PERSONNEL WITH CRITICAL MEDICAL INFORMATION IN A TIME OF CRISIS. COUNTLESS ADDITIONAL HOURS OF TELEPHONE SUPPORT WERE ALSO PROVIDED. THIS SERVICE IS PROVIDED AT NO COST, ALTHOUGH A DONATION IS GREATFULLY ACCEPTED.

CONGREGATE MEALS THROUGH THE CONGREGATE MEAL PROGRAM, THE COUNCIL ON AGING PROVIDES AN OPPORTUNITY FOR OLDER ADULTS, AGED 60 AND OVER, TO SHARE THEIR NOON MEAL WITH OTHER SENIORS GATHERED AT ANY ONE OF THREE CONGREGATE SETTINGS LOCATED ACROSS LAKE COUNTY, OHIO. MEALS ARE PREPARED FRESH DAILY. EACH OF THE MEALS IS REVIEWED BY A REGISTERED DIETICIAN THROUGH THE WESTERN RESERVE AREA AGENCY ON AGING TO ENSURE THAT THEY PROVIDE A MINIMUM OF ONE-THIRD OF THE DAILY RECOMMENDED NUTRITIONAL ALLOWANCES SENIORS NEED FOR GOOD HEALTH. ADDITIONALLY, PARTICIPANTS HAVE AN OPPORTUNITY TO SOCIALIZE WITH OTHERS AND CAN CHOOSE TO PARTAKE IN ACTIVITIES BEING OFFERED AT THE LOCATION. THE SETTING ALSO ALLOWS THE PARTICIPANTS TO BE CONNECTED TO OTHER COUNCIL ON AGING AND COMMUNITY SERVICES. IN 2019, 28,757 MEALS WERE SERVED TO 525 PARTICIPANTS. ALL MEALS ARE PROVIDED AT NO COST, ALTHOUGH A DONATION IS GREATFULLY ACCEPTED.
