Organizations Filed Purposes: TO ASSIST PEOPLE WITH DISABILITIES TO LIVE, WORK AND SOCIALIZE WITHIN A FULLY ACCESSIBLE COMMUNITY.

YOUTH AND TRANSITION SERVICES ARE OFFERED TO YOUTH AND THEIR FAMILIES TO SUPPORT INDEPENDENT LIVING GOALS THROUGH BEST PRACTICES AND PARTNERSHIPS WITH SCHOOLS, SERVICE AGENCIES, AND COMMUNITY ORGANIZATIONS. STAFF ENGAGE YOUTH IN ACTIVITIES THAT ALIGN WITH THEIR SKILLS AND INTERESTS INCLUDING RECREATION, EMPLOYMENT, SELF-ADVOCACY, AND TRANSITION. YOUTH CREATE A CUSTOMIZED PLAN INCLUDING TANGIBLE GOALS TO ACHIEVE GREATER INDEPENDENCE. INTERACTIVE LIFE SKILLS CLASSES ARE OFFERED TO YOUTH AND YOUNG ADULTS AGES 13 TO 26 LOOKING TO DEVELOP SKILLS FOR INDEPENDENT LIVING, COMPETITIVE EMPLOYMENT, OR COMMUNITY-BASED LIVING. CLASSES ARE FREE OF CHARGE AND RUN ON A SCHOOL CALENDAR YEAR. APPROXIMATELY 302 YOUTH AND THEIR FAMILIES WERE ASSISTED.

THE HOUSING RESOURCE CENTER ENCOMPASSES THREE DISTINCT PROGRAMS; ADAPTIVE EQUIPMENT LOAN AND ACQUISITION, HOME MODIFICATIONS AND COMMUNITY LIVING, TOGETHER PROVIDING SUPPORTS AND SERVICES FOR PEOPLE WITH DISABILITIES TO LIVE INDEPENDENTLY IN THE COMMUNITY OF THEIR CHOOSING. THE HOME MODIFICATION PROGRAM COMPLETED 129 HOME ACCESSIBILITY PROJECTS AND 773 CONSUMERS WERE EITHER LOANED OR GIFTED OVER A THOUSAND NEEDED PIECES OF MEDICAL EQUIPMENT. AN ASSISTIVE TECHNOLOGY PROGRAM WAS IMPLEMENTED IN AUGUST WITH 21 CONSUMERS BORROWING COMMUNICATION DEVICES. COMMUNITY LIVING SERVICES WERE PROVIDED TO 14 CONSUMERS BY ASSISTING IN TRANSITIONING FROM INSTITUTIONAL TO COMMUNITY-BASED LIVING.OUR ADVOCACY PROGRAM WORKS AT LOCAL, STATE, AND FEDERAL LEVELS TO PROMOTE POSITIVE CHANGE FOR PEOPLE WITH DISABILITIES. IN FY 2020, OUR ADVOCACY STAFF COMPLETED 79 ADVOCACY PROJECTS IN AREAS RELATED TO HEALCARE, TRANSPORTATION, HOUSING, AND ACCESSIBILITY.

THE ASSISTANCE DOGS PROGRAM GOAL IS TO HELP CHILDREN AND ADULTS WITH DISABILITIES ACHIEVE GREATER INDEPENDENCE BY TRANING AND PLACING SERVICE DOGS AND THERAPY DOGS TO ASSIST WITH THE INDIVIDUAL'S DAILY NEEDS. IN FY 2020, 19 SERVICE AND THERAPY DOGS WERE PLACED, BRINGING US TO 45 ADULT AND CHILD THERAPHY DOGS, 61 SERVICES DOGS, AND 23 SCHOOL THERAPY DOGS IN THE COMMUNITY.ACROSS ALL PROGRAMS, WE PROVIDE INFORMATION AND REFERRAL SERVICES. WE ANSWERED 6,693 REQUESTS FOR INFORMATION IN FY 2020.
