TO FOSTER QUALITY IMPROVEMENT IN GROUP PRACTICE THROUGH EDUCATION AND RESEARCH PROGRAMS IN CLINICAL QUALITY, PATIENT SAFETY, SERVICE, OPERATING EFFICIENCY AND INNOVATION.
THE TOGETHER 2 GOAL CAMPAIGN, LAUNCHED IN MARCH 2016, CHALLENGES PARTICIPATING MEDICAL GROUPS AND HEALTH SYSTEMS, PATIENTS AND THE ENTIRE NATION TO IMPROVE CARE FOR 1 MILLION PEOPLE WITH TYPE 2 DIABETES BY 2019. TO ACHIEVE THE AIM, THE CAMPAIGN LEVERAGES THE COORDINATED DELIVERY SYSTEMS OF NEARLY 140 PARTICIPATING AMGA, INC. MEMBERS AND CONCURRENTLY TAPS THE RESOURCES AND REACH OF NATIONAL PARTNERS AND CORPORATE COLLABORATORS. IN APRIL 2019, THIS CAMPAIGN WAS EXTENDED FOR ANOTHER 2 YEARS.
OBESITY CARE MODEL COLLABORATIVE-TWO COMMITTEES OF EXPERT ADVISORS DEVELOPED THE INCLUSION CRITERIA FOR SELECTION OF AMGA MEMBERS AND ARE DRAFTING THE FRAMEWORK AND MEASURES. THE 10 AMGA MEMBER GROUPS WERE SELECTED BASED ON THEIR ABILITY TO CONTRIBUTE TO THE FURTHER DEVELOPMENT OF THE FRAMEWORK, TEST MEASURES, AND COMMITMENT TO IMPLEMENT AN OBESITY PROGRAM THAT INCLUDES A MINIMUM RANGE OF SERVICES, EITHER WITHIN THE GROUP OR THROUGH REFERRAL PARTNERSHIP. PARTICIPATING ORGANIZATIONS WILL MEET IN-PERSON FOUR TIMES AND ATTEND MONTHLY EDUCATIONAL WEBINARS TO SHARE INSIGHTS AND CHALLENGES AND TO REFINE THE FRAMEWORK AND MEASURES BASED ON THE DATA AND THEIR EXPERIENCES.
T2G BUNDLE MEASURE -TEN AMGA MEMBER ORGANIZATIONS PARTICIPATING IN THE T2G CAMPAIGN WERE SELECTED TO PARTICIPATE IN A SUPPLEMENTAL, INTENSIVE LEARNING COLLABORATIVE WHICH WILL PROVIDE ADDITIONAL RESOURCES TO IMPROVE THE CARE OF YOUR PATIENTS WITH TYPE 2 DIABETES. THE DIABETES BUNDLE COLLABORATIVE PROVIDES THE OPPORTUNITY TO AUGMENT THEIR QUALITY IMPROVEMENT WORK AND ACCELERATE THE PACE OF IMPROVEMENT. THIS COLLABORATIVE IS AN EXPANSION OF AMGA'S SUCCESSFUL SIX-MONTH PILOT INITIATIVE, WHICH SOUGHT TO IMPROVE OUTCOMES ON THE T2G CORE TRACK BUNDLE MEASURE.
