HOSPICE OF THE RAPIDAN PROVIDES HIGH QUALITY, COMPASSIONATE END-OF-LIFE MEDICAL CARE FOR PEOPLE WITH TERMINAL ILLNESSES IN THE COUNTIES OF CULPEPER, FAUQUIER, MADISON, ORANGE AND RAPPAHANNOCK, VIRGINIA. HOSPICE CARE FOCUSES ON CARING, NOT CURING. THE PREMISE OF HOSPICE IS THAT EACH OF US IS ENTITLED TO DIE PAIN FREE AND WITH DIGNITY, SURROUNDED BY FAMILY AND LOVED ONES WITH THE SUPPORT OF CARING PEOPLE WHO UNDERSTAND THE PROCESS OF DYING PEACEFULLY.
IN THE FISCAL YEAR OF 2014-2015 HOSPICE OF THE RAPIDAN CARED FOR 140 PATIENTS PROVIDING 4,606 TOTAL DAYS OF CARING.HOSPICE OF THE RAPIDAN IS A CENTRALLY ADMINISTERED PROGRAM OF COORDINATED HOME CARE AND INPATIENT SERVICES PRIMARILY FOR TERMINALLY ILL PATIENTS IN THE LAST SEVERAL MONTHS OF LIFE. PRIMARY EMPHASIS IS PLACED ON KEEPING THE END STAGE PATIENT COMFORTABLE AT HIS OR HER PLACE OF RESIDENCE. HOSPICE SERVICES ARE PROVIDED IN A MANNER CONSISTENT WITH ACCEPTABLE STANDARDS OF PRACTICE AND IN COMPLIANCE WITH FEDERAL, STATE AND PRIVATE INSURER'S STANDARDS.THE PATIENT, THE PRIMARY CAREGIVER AND THE PATIENT'S FAMILY ARE CONSIDERED A UNIT OF CARE. THE PATIENT AND THE FAMILY ARE ACTIVELY ENCOURAGED TO MAKE DECISIONS ABOUT HOSPICE CARE.A DESIGNATED INTERDISCIPLINARY GROUP SUPERVISES THE PATIENT'S CARE AND FUNCTIONS PRIMARILY TO MAXIMIZE THE PATIENT'S COMFORT BY SYMPTOM MANAGEMENT ACCORDING TO A PLAN OF CARE ESTABLISHED BY THE ATTENDING PHYSICIAN OR PHYSICIAN DESIGNEE AND THE MEDICAL DIRECTOR.SERVICES ARE AVAILABLE 24 HOURS, 7 DAYS A WEEK.INPATIENT CARE IS PROVIDED IN AN ATMOSPHERE AS HOME LIKE AS PRACTICAL.BEREAVEMENT FOLLOW UP IS PROVIDED. INDIVIDUAL AND GROUP SUPPORT IS EXTENDED TO FAMILIES IN GRIEF IN THE FIRST 12 MONTHS FOLLOWING THE PATIENT'S DEATH.THE NON-SALARIED EMPLOYEES (VOLUNTEERS) ARE TRAINED TO DO SPECIFIC JOB FUNCTIONS WITHIN THE HOSPICE SERVICE DELIVERY SYSTEM.SERVICES ARE BASED ON THE PATIENT'S NEEDS. HOSPICE SERVICES ARE AVAILABLE TO ALL ELIGIBLE PATIENTS WITHOUT REGARD TO THE ABILITY TO PAY FOR SUCH SERVICES.THE FOLLOWING IS A LIST OF SERVICES AVAILABLE TO HOSPICE OF THE RAPIDAN PATIENTS AS NEEDED AND APPROPRIATE BASED ON THE PLAN OF CARE DEVELOPED BY THE INTERDISCIPLINARY TEAM INCLUDING THE ATTENDING PHYSICIAN. -NURSING SERVICES -MEDICAL/SOCIAL WORK -VOLUNTEER SERVICES -PHYSICAL, OCCUPATIONAL, SPEECH THERAPY, DIETARY SERVICES -HOME HEALTH AIDE -SHORT TERM INPATIENT SERVICES -PHARMACEUTICAL SERVICES -MEDICAL APPLIANCES AND SUPPLIES, INCLUDING DRUGS AND BIOLOGICAL RELEVANT TO THE PATIENT'S TERMINAL ILLNESS -RESPITE SERVICES -PHYSICIAN SERVICES -SPIRITUAL SUPPORT AND COUNSELING SERVICES -BEREAVEMENT AND COUNSELING SUPPORT
