TO SUPPORT OLDER ADULTS AND ADULTS WITH DISABILITIES TO LIVE WITH DIGNITY IN ACCORDANCE WITH THEIR OWN PERSONAL CHOICE.

SENIOR SUPPORTIVE SERVICES PROGRAM:WE PROVIDED 6,423 INFORMATION CONTACTS AND REFERRALS AND 194 HOURS OF CARE COORDINATION SERVICES TO SENIORS IN ORDER FOR THEM TO LOCATE APPROPRIATE SERVICES TO ENSURE THEY RECEIVE THE ASSISTANCE NEEDED TO REMAIN IN THEIR OWN HOMES. WE PROVIDED ELDER ABUSE PREVENTION SERVICES, INCLUDING ADVOCACY AND OMBUDSMAN ACTIVITIES, WHICH ENCOMPASSED IN-SERVICE TRAINING AT LONG-TERM CARE FACILITIES AND LONG-TERM CARE FACILITY MEDIATION SERVICES FOR THE ELDERLY AND THEIR FAMILIES. ON 13,139 SEPARATE OCCASIONS, WE TRANSPORTED SENIORS, WHO HAD NO OTHER MEANS OF TRANSPORTATION, TO AND FROM DOCTORS, DENTISTS, MEDICAL TREATMENT FACILITIES, BANKS, GROCERS, AND OTHER DESTINATIONS, INCLUDING OUR SITES FOR PARTICIPATION IN OUR MEALS PROGRAM AND VARIOUS PLANNED ACTIVITIES. OUR SITE PARTICIPANTS RECEIVED DISEASE PREVENTION COUNSELING AND HEALTH SCREENINGS. IN ADDITION, SENIORS PARTICIPATED IN 2,990 HOURS OF RECREATION AND SOCIAL ACTIVITIES AT OUR VARIOUS SITES. ALONG WITH 1,580 HOURS OF VOLUNTEER OPPORTUNITIES, SERVICES WE PROVIDED INCLUDED SERVICE ON LOCAL LONG-TERM COMMITTEES AND PRESENTATIONS REGARDING THE SERVICES WE PROVIDE. UTILIZING A LOCAL LEGAL AID ORGANIZATION, WE PROVIDED 37 HOURS OF LEGAL SERVICES TO SENIORS WHO WERE NOT OTHERWISE ABLE TO AFFORD SUCH SERVICES. THE ALLIANCE PROVIDED 4,857 CONTACTS CHECKING INDIVIDUALS AND 7,950 HOURS FOR PERSONAL CARE

CONGREGATE AND HOME DELIVERED NUTRITIOUS MEALS PROGRAM:DURING THE CURRENT YEAR, INDIVIDUALS WERE PROVIDED WITH 90,298 NUTRITIONALLY BALANCED MEALS. ALL MEALS MET ONE-THIRD OF THE RECOMMENDED DIETARY ALLOWANCE (RDA). MEALS WERE HOME DELIVERED OR PROVIDED AT A NUTRITION EDUCATION SITE. INDIVIDUALS ATTENDING A NUTRITION EDUCATION SITE WERE PROVIDED TRANSPORTATION, HEALTH AND NUTRITION EDUCATION, HEALTH SCREENINGS AND RECREATION.

WEATHERIZATION AND LOW-INCOME HOUSING ENERGY ASSISTANCE PROGRAM:THIS PROGRAM IS DESIGNED TO ASSIST SENIORS IN MAKING THEIR SITE-BUILT OR MOBILE HOMES MORE ENERGY EFFICIENT, THEREBY REDUCING THEIR HEATING AND COOLING COSTS, AND ENABLING THEM TO LIVE MORE COMFORTABLY. ASSISTANCE PROVIDED INCLUDED INSULATION OF HOMES; REPAIR OR REPLACEMENT OF FURNACES; AND SEALING DUCTWORK, WINDOWS, AND DOORS. DURING THE CURRENT YEAR, 46 HOMES WERE WEATHERIZED AND/OR REPAIRED THROUGH THIS PROGRAM.
