THE ORGANIZATION'S MISSION IS TO PROVIDE THE PROGRAMS AND SERVICES NECESSARY FOR PEOPLE OF ALL AGES TO LIVE INDEPENDENTLY IN THEIR COMMUNITIES FOR AS LONG AS POSSIBLE. EVERY PERSON WILL HAVE A CHOICE AND RANGE OF SERVICES THAT WILL ASSIST THEM TO REMAIN INDEPENDENT IN THEIR CHOSEN HOME.
THE ORGANIZATION DELIVERS PROGRAMS AND SERVICES TO PEOPLE OF ALL AGES TO HELP THEM LIVE INDEPENDENTLY IN THEIR COMMUNITIES FOR AS LONG AS POSSIBLE.
THE COMMUNITY LIVING PROGRAM OFFERS A RANGE OF PROGRAMS AND SERVICES THAT BENEFIT PEOPLE OF ALL AGES. SERVING MORE THAN 23,000 INDIVIDUALS IN FISCAL 2019, THESE PROGRAMS INCLUDE CONGREGATE AND HOME DELIVERED MEALS; 8 ACTIVE LIFESTYLE CENTERS (SENIOR CENTERS); 2 ADULT DAY CARE CENTERS PROVIDING RESPITE CARE; IN-HOME PERSONAL CARE, COMPANION CARE AND COORDINATED CARE SERVICES; VETERAN-DIRECTED CARE FINANCIAL MANAGEMENT SERVICES AND COUNSELING; CARE TRANSITIONS INTERVENTION DESIGNED TO REDUCE HOSPITAL READMISSIONS; SENIOR EMPLOYMENT TRAINING AND INSURANCE COUNSELING, INFORMATION AND REFERRAL SERVICES, AND OTHER ACTIVITIES. 845 VOLUNTEERS PROVIDED 17,965 HOURS OF SERVICE PROVIDING NEARLY 145,000 MEALS, NON- EMERGENCY MEDICAL RIDES, INSURANCE COUNSELING, FRIENDLY CALLS AND VISITS, AND OTHER ACTIVITIES.
BAY TRANSIT PROVIDES PUBLIC TRANSPORTATION FOR ALL PEOPLE OF ALL AGES. ACROSS OUR 12-COUNTY SERVICE AREA WE PROVIDED 143,106 RIDES IN FISCAL 2019. RIDERS UTILIZED BAY TRANSIT TO ACCESS EMPLOYMENT, HEALTHCARE, SHOPPING, AND NUMEROUS OTHER CONSUMER SERVICES. THE NEW FREEDOM AND MED-CARRY PROGRAMS SERVED 621 PEOPLE WITH MORE THAN 4,800 RIDES TO CRITICAL HEALTHCARE SERVICE DESTINATIONS BEYOND OUR STANDARD SERVICE AREA.
BAY AGING SENIOR APARTMENTS, WHICH ARE HUD SUBSIDIZED PROPERTIES, PROVIDE SAFE, AFFORDABLE HOUSING AND ENRICHED SUPPORTS TO 366 OCCUPANTS AGE 62 AND OLDER RESIDING IN 10 APARTMENT COMMUNITIES LOCATED IN THE MIDDLE PENINSULA AND NORTHERN NECK. THE HOUSING CHOICE VOUCHER PROGRAM ALLOWED 570 PEOPLE OF ALL AGES ACCESS TO 272 SAFE, STABLE HOMES. SINGLE-FAMILY HOUSING ENCOMPASSES 4 MAJOR INITIATIVES - WEATHERIZATION, INDOOR PLUMBING REHABILITATION, EMERGENCY HOME REPAIR AND COMMUNITY DEVELOPMENT BLOCK GRANTS. THESE PROGRAMS UPDATED 145 HOMES, ALLOWING 310 PEOPLE TO LIVE IN MORE HEALTHY, STABLE HOMES.
