THE ORGANIZATION EDUCATES THE PUBLIC ON THE VALUE OF HOME HEALTH SERVICES AND ASSISTS CAREGIVERS THROUGH EDUCATION OF CAREGIVING SKILLS, KNOWLEDGE, AND RESOURCES TO IMPROVE THE CARE FOR THE PATIENT AND ALLEVIATE THE STRESS OF THE CAREGIVER. THE ORGANIZATION PARTICIPATES IN COMMUNITY EDUCATION AND PROVISION OF SERVICES TO REDUCE THE RISK OF INFECTION INCLUDING THE PROVISION OF FLU VACCINES.

WESLEY HOMES AT HOME PROVIDES INTERMITTENT, PART-TIME, NURSING, PHYSICAL, OCCUPATIONAL, AND SPEECH THERAPY, MEDICAL SOCIAL SERVICES, AND HOME HEALTH AIDE SERVICES TO PATIENTS IN THEIR OWN HOMES. THE ORGANIZATION EDUCATES THE PUBLIC ON THE VALUE OF HOME HEALTH SERVICES AND ASSISTS CAREGIVERS THROUGH EDUCATION OF CAREGIVING SKILLS, KNOWLEDGE, AND RESOURCES TO IMPROVE THE CARE FOR THE PATIENT AND ALLEVIATE THE STRESS OF THE CAREGIVER. THE ORGANIZATION PARTICIPATES IN COMMUNITY EDUCATION AND PROVISION OF SERVICES TO REDUCE THE RISK OF INFECTION INCLUDING THE PROVISION OF FLU VACCINES.

WESLEY HOMES HOSPICE PROVIDES END-OF-LIFE AND PALLIATIVE CARE TO PATIENTS IN THEIR HOMES. INTERMITTENT AND HOURLY CARE IS PROVIDED BY THE INTERDISCIPLINARY TEAM OF NURSES, SOCIAL WORKERS, HOSPICE AIDES, A CHAPLAIN, VOLUNTEERS, PHARMACISTS, A MEDICAL DIRECTOR, AND PHYSICAL, OCCUPATIONAL, AND SPEECH THERAPY. THE TEAM ADDRESSES PAIN AND SYMPTOM MANAGEMENT, BEREAVEMENT SERVICES, AND SPIRITUAL COUNSELING OF THE PATIENT AND LOVED ONES. THE ORGANIZATION EDUCATES THE PUBLIC ON THE PURPOSE AND BENEFITS OF HOSPICE AND PALLIATIVE CARE, END-OF-LIFE PLANNING AND DECISION-MAKING, AND PROVIDES SUPPORT FOR THE CAREGIVER THROUGH EDUCATION REGARDING THE END-OF-LIFE PROCESS, RESOURCES TO REDUCE STRESS, AND ACCESS TO RESPITE SERVICES.
