TRANSPORTATION SERVICES: WE BELIEVE THAT EVERYONE SHOULD HAVE SAFE AND ACCESSIBLE TRANSPORTATION OPTIONS THAT CONNECT THEM TO HEALTH CARE, SERVICES, AND THEIR COMMUNITY. THROUGH A CONTRACT WITH COMMUNITY TRANSIT, DIAL-A-RIDE TRANSPORTATION (DART) PROVIDED 178,691 TRIPS WITH 52 WHEELCHAIR ACCESSIBLE VEHICLES OPERATING SEVEN DAYS A WEEK. FOR PEOPLE WHO DO NOT MEET ELIGIBILITY REQUIREMENTS FOR DART, THE TRANSPORTATION ASSISTANCE PROGRAM (TAP) PROVIDED 20,383 TRIPS WITH ELEVEN WHEELCHAIR ACCESSIBLE VEHICLES AND ONE STANDARD MINIVAN OPERATING FIVE DAYS A WEEK.
SOCIAL SERVICES: WE BELIEVE THAT EVERYONE SHOULD HAVE ACCESS TO INFORMATION AND SERVICES THAT PROMOTE INDEPENDENCE AND WELLNESS. INFORMATION AND ASSISTANCE (AGING AND DISABILITY RESOURCES) RESPONDED TO 19,667 REQUESTS FOR INFORMATION THAT CONNECTED PEOPLE TO THE RIGHT SERVICE AT THE RIGHT TIME. STATEWIDE HEALTH INSURANCE BENEFITS ADVISORS (SHIBA) HELPED 5,368 PEOPLE MAKE INFORMED DECISIONS ABOUT THEIR HEALTH INSURANCE. FAMILY CAREGIVER SUPPORT ANSWERED 338 REQUESTS FOR HELP WITH IN-HOME CAREGIVING. MULTICULTURAL OUTREACH AND THE MULTICULTURAL SENIOR CENTER BENEFITED 607 OLDER ADULTS WITH FOOD, FRIENDSHIP, AND OTHER CRITICAL SERVICES AT THE CENTER FOR HEALTHY LIVING. DEPRESSION SCREENING PROVIDED EDUCATION, SCREENING, AND COUNSELING TO 234 SENIORS. CARE COORDINATION SUPPORTED 188 PEOPLE WHO HAVE COMPLEX HEALTH NEEDS WITH MULTIPLE HOME VISITS. MENTAL HEALTH ACCESS PROVIDED INFORMATION TO 356 CALLERS REQUESTING MENTAL HEALTH ASSISTANCE. 64 SENIORS PARTICIPATED IN CHRONIC DISEASE SELF-MANAGEMENT CLASSES. SENIOR PEER COUNSELING VOLUNTEERS SUPPORTED 93 SENIORS WHO EXPERIENCED LOSS AND OTHER AGE-RELATED ISSUES.
NUTRITION SERVICES: WE BELIEVE THAT GOOD NUTRITION AND SOCIALIZATION IMPROVE HEALTH AND QUALITY OF LIFE. MEALS ON WHEELS DELIVERED 221,950 MEALS TO 1,455 PEOPLE WHO ARE HOMEBOUND AND NO LONGER ABLE TO COOK FOR THEMSELVES. SENIOR DINING PROVIDED 69,165 MEALS TO 2,213 PEOPLE AT TWELVE SENIOR MEAL SITES. FARMER'S MARKET PROVIDED FRESH PRODUCE VOUCHERS TO 1,822 LOW-INCOME SENIORS.
