FOR 2019, COMMUNITY-BASED HEALTHY PROGRAM ENCOMPASSED WORK RELATED TO TACOMA-PIERCE COUNTY, WA DOH (COALITION, CHW TASKFORCE, OBJ A, C, D OF THE DOH'S 2019 CONTRACT).COALESCING FOR CHANGE - FOR EFFECTIVE POLICY AND SYSTEMS CHANGE WE CONNECT WITH LEADERS FROM A VARIETY OF SECTORS AND DISCIPLINES AND INFORM THEM ABOUT THEIR OVERLAPPING INTEREST AND INVESTMENTS; ANALYZE DATA AND RESEARCH TO SUPPLY FINDINGS TO DECISION MAKERS AT ALL LEVELS OF HEALTH IMPROVEMENT EFFORTS TO INFORM HOW THEY DESIGN POLICY AND IMPLEMENT INNOVATIONS. THIS STRATEGIC SUPPORT IS IN THE FORM OF FUNDS AND TECHNICAL EXPERTISE TO SUPPORT THE WORK RELATED TO THE PREVENTION ALLIANCE, AND COMMUNITY HEALTH WORKERS (CHWS).

THE PREVENTION ALLIANCE INCUBATES PSE (POLICY, SYSTEMS AND ENVIRONMENTAL) STRATEGIES FOR LOCAL AND STATEWIDE HEALTH IMPROVEMENTS. WE COORDINATE THIS BROAD COLLABORATION BETWEEN CROSS SECTOR GROUPS REPRESENTING TRANSPORTATION, HEALTHY EATING, TOBACCO AND VAPE PREVENTION, SCHOOLBASED CLINICS, HOUSING, AND LOCAL HEALTH DEPARTMENTS. FOCUS ON CONNECTING MEETING THE NEEDS OF THE DIVERSE COMMUNITY HEALTH WORKER WORKFORCE BY CONTINUOUSLY IMPROVING TRAINING, PROFESSIONAL DEVELOPMENT, SUCH AS TRAINING CHWS IN DIABETES, HYPERTENSION AND HEALTHY COOKING. ESTIMATED IMPACT: 2 MILLION WASHINGTON RESIDENTS STATEWIDE. SINCE 2011 FOUNDATION FOR HEALTHY GENERATIONS HAS SUPPORTED A COMMUNITY HEALTH ADVOCATE (CHA) PROGRAM AT SALISHAN, A MIXED INCOME HOUSING DEVELOPMENT IN TACOMA, WA.

THE SALISHAN CHAS ARE NEIGHBORHOOD-BASED LEADERS WITH A STRONG COMMITMENT TO - AND CONNECTION WITH - THEIR COMMUNITIES. ADVOCATES ARE NATURAL HELPERS WHO ARE INVESTED IN ELEVATING COMMUNITY VOICE AND IMPROVING HEALTH OUTCOMES. THROUGH THEIR TRAINING AND SUPERVISION, ADVOCATES ARE CONTINUALLY LEARNING ABOUT HEALTH AND WELLNESS. THEY ARE ALSO ENCOURAGED TO ENGAGE IN HEALTHY BEHAVIORS THEMSELVES, AND SUPPORT THOSE CLOSE TO THEM IN EVERYDAY BEHAVIORS THAT PROMOTE GOOD HEALTH. EXAMPLES OF ADVOCATES ENGAGING OTHERS INCLUDE: PARTICIPATING IN WALKING GROUPS, COMMUNITY GARDENING, TRYING NEW HEALTHY FOODS, WEEKLY BLOOD PRESSURE CHECKS, PARTICIPATING IN COMMUNITY EVENTS AND GOING TO MEDICAL APPOINTMENTS WITH THEIR NEIGHBORS. ESTIMATED IMPACT: 3,200.
