HOME CARE SERVICESFULL LIFE CARE PROVIDED IN HOME CARE SERVICES TO HUNDREDS OF CLIENTS WITH OVER 350 HOME CARE AIDES. THIS SERVICE ALLOWS THE CLIENT TO REMAIN IN THEIR HOME FOR AS LONG AS POSSIBLE AND AVOID INSTITUTIONAL CARE OF THE RISK OF HOMELESSNESS.ADULT DAY SERVICESFULL LIFE CARE PROVIDED ADULT DAY HEALTH CARE TO OVER 875 FRAIL ELDERS AND ADULTS WITH DISABILITIES AT 4 LOCATIONS IN KING AND SNOHOMISH COUNTIES. SERVICES INCLUDED SKILLED NURSING, REHABILITATIVE THERAPY, AND SOCIAL PROGRAMS WHICH HELPED PERSONS WITH CHRONIC HEALTH CONDITIONS SUCH AS STROKE, DEMENTIA, ACQUIRED BRAIN INJURY, AND DEVELOPMENT DISABILITIES AVOID LIFE IN A NURSING HOME OR OTHER INSTITUTIONAL SETTING. THESE ADULT DAY HEALTH PROGRAMS PROVIDED RESPITE TO HUNDREDS OF FAMILY CARE GIVERS.BEHAVIORAL HEALTHOUR SOLSTICE PROGRAM OFFERS TREATMENT, COUNSELING AND CASE MANAGEMENT SUPPORT TO PEOPLE LIVING WITH MENTAL HEALTH ISSUES. LICENSED CLINICIANS WORK WITH INDIVIDUALS TO ADDRESS AN ARRAY OF ISSUES SUCH AS DEPRESSION, PTSD, HOARDING DISORDER, OR SCHIZOPHRENIA. THERAPEUTIC ART AND WELLNESS STUDIOS. TREATMENT IS OFFERED IN INDIVIDUAL AND GROUP SETTINGS, INCLUDING THERAPEUTIC ART AND WELLNESS STUDIOS. HOUSING SUPPORTSONGOING SUPPORTS TO HELP PEOPLE FIND AND KEEP STABLE, INDEPENDENT HOUSING. SERVICES INCLUDE: HOUSING ASSESSMENTS, IDENTIFYING HOUSING RESOURCES, SUPPORT OBTAINING A LEASE, INDEPENDENT LIVING SKILLS DEVELOPMENT, LANDLORD RELATIONS, AND CRISIS MANAGEMENT. CARE CONSULTATIONPROVIDES INDIVIDUALIZED ASSISTANCE FOR INDIVIDUALS AND FAMILIES TO NAVIGATE CARE OPTIONS, IMPLEMENT AND MONITOR CARE SERVICES AND APPLY FOR BENEFITS IF ELIGIBLE. LEVEL OF ASSISTANCE IS BASED ON NEED: RANGES FROM A ONE-TIME CONSULTATION TO ONGOING CARE MANAGEMENT.
