The mission of POCAAN is to work with communities of color and other minority communities to advocate and ensure access to HIV/AIDS education, prevention programs support and health care resources for people of color.
POCAAN has long southt to reduce disparities experienced by clients and improve health equity through enhanced systems navigation and trauma-informed approaches to service that are client directed. This year POCAAN served more than 1,200 clients with funding through our housing programs for the homeless, Homeless Prevention, prison re-entry, substance abuse prevention, HIV testing/prevention education and referral programs, Blak PRIDE community outreach, case management and primary care services. POCAAN has developed, nurtured and sustained collaborations with many state, County, and national entities to do this work.
