EXCELSIOR YOUTH CENTER PROVIDES COMPREHENSIVE SERVICES TO MEET THE HOLISTIC NEEDS OF THOSE WE SERVE.OUR STAFF SEEKS FORMAL AND INFORMAL EDUCATIONAL OPPORTUNITIES TO DEVELOP SKILL SETS THAT ENHANCE THE RELATIONSHIPS WE BUILD WITH CLIENTS. WE ESTABLISH A PROFESSIONAL ENVIRONMENT MARKED BY COLLABORATION, RESPECT, AND UNITY. WE ARE IN THE PROCESSES OF CONTINUED SELF-EVALUATION AND SELF-IMPROVEMENT. EXCELSIOR'S WRAP AROUND INTENSIVE SERVICE PROGRAM (WISE), PROVIDES MENTAL HEALTH, CARE COORDINATION, PEER SUPPORT AND THERAPEUTIC RECREATION ACTIVITIES TO MEDICAID ELIGIBLE YOUTH AND FAMILIES IN THEIR HOME AND IN COMMUNITY SETTINGS. INTEGRATED ACCESS TO MEDICAL AND EDUCATION SERVICES ARE AT THE PROGRAMS DISPOSAL. AVERAGE CLIENTS PER MONTH 95.

LIFE POINT PROVIDES COMPREHENSIVE TRANSITION SERVICES. THE PROGRAM PROVIDES HOSPITAL DIVERSION FROM ACUTE INSTITUTIONAL CARE FOR YOUTH WHO LACK SUPPORT SYSTEM RESOURCES, AND STRUGGLE WITH HOMELESSNESS, MENTAL ILLNESS, SUBSTANCE USE AND PHYSICAL HEALTH DIFFICULTIES. THIS PROGRAM UTILIZES AND INTENSIVE WRAPAROUND APPROACH TO DELIVER SUPPORT SERVICES FROM AREAS NEEDED TO ESTABLISH HEALTH AND CONFIDENCE TO LIVE INDEPENDENTLY. HOUSING, HEALTHCARE, FOSTER CARE, AND EDUCATION FUNDING IS BRAIDED TO MEET HOLISTIC NEEDS OF THIS TRANSITION AGE YOUTH (TAY) POPULATION. ALL LIFE POINT SERVICES ARE INTEGRATED WITH ONSITE PRIMARY MEDICAL AND EDUCATIONAL SERVICES. AVERAGE CLIENTS PER MONTH 95.

OLYMPUS PROVIDES SERVICES TO INDIVIDUALS PLACED FOR BEHAVIORAL HEALTH SERVICES AND SUBSTANCE USE INTENSIVE INPATIENT. TYPICAL PATIENTS HAVE EXPERIENCED SIGNIFICANT TRAUMATIZING EVENTS (ADVERSE CHILDHOOD EXPERIENCE-ACES) AND OFTEN AN ABSENCE OF ANY FAMILIAL SUPPORT SYSTEM RESOURCES. THESE YOUTH RECEIVE HOLISTIC, TRAUMA-INFORMED, AND EVIDENCE-BASED CARE. LENGTHS OF STAY RANGE FROM BRIEF INTERVENTION TO LONG-TERM PSYCHIATRIC RESIDENTIAL CARE. ALL INPATIENT SERVICES ARE INTEGRATED WITH ONSITE PRIMARY MEDICAL AND EDUCATIONAL SERVICES. TOTAL BED DAYS 3,055.
