HOSPICE OF ST. FRANCIS PROVIDES COMPREHENSIVE HOSPICE CARE SERVICES AND SERVED THE NEEDS OF 1,662 PATIENTS THIS YEAR WITH AN AVERAGE DAILY CENSUS OF 388. THE MAJORITY OF CARE IS PROVIDED IN PATIENTS' RESIDENCES, INCLUDING PRIVATE HOMES, ASSISTED LIVING FACILITIES, AND SKILLED NURSING FACILITIES. HOSPICE CARE IS ALSO PROVIDED IN THE HOSPITAL AND IN OUR EDWARD M. POE HOSPICE CARE CENTER IN TITUSVILLE. A COMPREHENSIVE, INTERDISCIPLINARY TEAM IS RESPONSIBLE FOR A HIGHLY COORDINATED HOSPICE CARE EXPERIENCE FOR BOTH PATIENTS AND FAMILIES.
HOSPICE OF ST. FRANCIS OFFERS A ROBUST PALLIATIVE CARE PROGRAM CALLED ST. FRANCIS PATHWAYS TO HEALTHCARE. THIS INITIATIVE ADDRESSES THE SYMPTOMS & SIDE EFFECTS OF SUCH LIFE-LIMITING MEDICAL CONDITIONS AS CANCER, FAR-ADVANCED HEART, LUNG, KIDNEY & LIVER DISEASE, NEUROLOGICAL DISORDERS, DEMENTIA, & MORE. WHEN THE FIRST IMPACT OF CHRONIC DEBILITATIVE DISEASE IS EXPERIENCED, PALLIATIVE CARE CAN HELP PATIENTS & THEIR CAREGIVERS TO SUCCESSFULLY MANAGE THEIR SELF-CARE TO MINIMIZE PROBLEMS THAT MAKE IT DIFFICULT TO PERFORM ACTIVITIES OF DAILY LIVING OR SEEK CURATIVE TREATMENT. PALLIATIVE CARE CAN ALSO CAN MINIMIZE TRIPS TO THE EMERGENCY ROOM & HOSPITAL ADMISSIONS WHILE SUPPORTING PATIENTS' GOALS FOR THEIR CARE & QUALITY OF LIFE. ST. FRANCIS PATHWAYS TO HEALTHCARE SERVED 660 PATIENTS IN FISCAL YEAR 2019. PATHWAYS PROVIDES SUPPORT FOR PATIENTS & CAREGIVERS THROUGHOUT THE CONTINUUM OF CARE, INCLUDING TIMES OF TRANSITION, & COLLABORATES WITH COMMUNITY CARE PARTNERS TO PROVIDE THE BEST SERVICES TO PATIENTS AND FAMILIES.
HOSPICE OF ST. FRANCIS OFFERS A DYNAMIC GRIEF SUPPORT PROGRAM THAT IS OPEN TO THE PUBLIC COUNTY-WIDE, REGARDLESS OF WHETHER HOSPICE SERVICE WAS INVOLVED IN THE CARE OF THE DECEASED INDIVIDUAL. IN 2019, THE BEREAVEMENT DEPARTMENT PROVIDED SUPPORT TO 2,160 BEREAVED FAMILIES THROUGH INDIVIDUAL OR GROUP SESSIONS AND SUPPORT ACTIVITIES, AS WELL AS ENGAGING IN SIGNIFICANT OUTREACH THROUGH MONTHLY MAILINGS AND PHONE CALLS. 300 ADDITIONAL INDIVIDUALS IN BREVARD UTILIZED OUR BEREAVEMENT SERVICES, EVEN THOUGH THEIR LOSS WAS NOT ASSOCIATED WITH OUR HOSPICE CARE. THE BEREAVEMENT TEAM PRESENTED INFORMATIONAL TALKS TO VARIOUS COMMUNITY ORGANIZATIONS, INCLUDING NURSING FACILITIES, CHURCHES, EDUCATIONAL INSTITUTIONS AND COMMUNITY GATHERING SITES. THROUGH THESE TALKS, GRIEF EDUCATION WAS PROVIDED TO AN ADDITIONAL 350 PEOPLE. FOUR MEMORIAL SERVICES WERE CONDUCTED AT OUR OFFICES TO OFFER SURVIVING LOVED ONES A CHANCE TO HONOR AND REMEMBER THOSE PATIENTS CARED FOR BY HOSPICE OF ST. FRANCIS. IN ALL, APPROXIMATELY 200 FAMILY MEMBERS, FRIENDS AND HOSPICE OF ST. FRANCIS STAFF ATTENDED. THREE COMMUNITY MEMORIAL SERVICES WERE HELD TO SUPPORT THOSE COPING WITH LOSS AT THE START OF THE HOLIDAY SEASON, SERVING AN ADDITIONAL 450 PEOPLE THROUGH THOSE SERVICES. THE SPECIAL NEEDS OF GRIEVING CHILDREN AGES 4 TO 18 WERE SERVED BY OUR NORTH STAR FOR GRIEVING CHILDREN PROGRAM WHICH PROVIDES SAFE AND CARING PEER SUPPORT GROUPS FACILITATED BY MASTER'S LEVEL COUNSELORS AND TRAINED VOLUNTEERS. THIS PROGRAM IS OFFERED FREE OF CHARGE TO ANY CHILD IN BREVARD COUNTY WHO HAS SUFFERED A LOSS. IN ADDITION, NORTH STAR STAFF PROVIDED EDUCATIONAL WORKSHOPS ON CHILD GRIEF TO BREVARD COUNTY SCHOOL BOARD EMPLOYEES, INCLUDING TEACHERS AND COUNSELORS, AND RESPONDED TO SCHOOL COMMUNITIES COPING WITH THE SUDDEN LOSS OF A TEACHER OR STUDENT. HOSPICE OF ST. FRANCIS IS PROUD OF THE EXTENSIVE SCOPE OF FREE BEREAVEMENT SERVICES IT OFFERS, RECOGNIZING THAT DEALING WITH GRIEF IS A COMMON CHALLENGE IMPACTING PEOPLE OF ALL AGES AND WALKS OF LIFE.
