Organizations Filed Purposes: THROUGH PROGRAMS, ADVOCACY AND COLLABORATION, PEOPLE ARE EMPOWERED TO DISCOVER THEIR POTENTIAL AND LIVE AS ENGAGED CITIZENS, FREE OF STIGMA, WITHIN A THRIVING COMMUNITY.

WE SUPPORT IND. AND FAMILIES IN THEIR EFFORTS TO MEET ECONOMIC, SOCIAL AND EMOTIONAL CHALLENGES.

COMMUNITY SUPPORT AND RECOVERY PROGRAMS -THE COMMUNITY SUPPORT PROGRAM (CSP) PROVIDES CLINICAL AND SUPPORT SERVICES FOR INDIVIDUALS WITH LONG-TERM NEEDS ASSOCIATED WITH MENTAL ILLNESS AND/OR SUBSTANCE USE, WITH THE GOAL TO HELP PEOPLE LIVE AS INDEPENDENTLY AND AS FULLY INTEGRATED WITHIN THE COMMUNITY AS POSSIBLE, AND TO REDUCE HOSPITALIZATIONS AND INSTITUTIONAL CARE. 943 INDIVIDUALS WERE ENROLLED IN CSP WITH 459 DIAGNOSED WITH CO-OCCURRING SUBSTANCE USE ISSUES. 755 WERE ENROLLED IN AN INTEGRATED HEALTH HOME, AND 174 WERE ENROLLED IN ASSERTIVE COMMUNITY TREATMENT. FOUR MENTAL HEALTH PSYCHIATRIC REHABILITATIVE RESIDENCES PROVIDE TRANSITIONAL PLACEMENT FOR THOSE DISCHARGED FROM LONG-TERM HOSPITALIZATION AND REQUIRING 24/7 SUPPORT TO DEVELOP SKILLS NEEDED TO LIVE INDEPENDENTLY. 48 CLIENTS WERE SERVED BY MHPRR SERVICES. THE WELLNESS AND RECOVERY CENTER OFFERS THERAPEUTIC, HEALTH AND WELLNESS AND PEER SUPPORT GROUPS TO CLIENTS OF THE COMMUNITY SUPPORT PROGRAM. 141 CLIENTS PARTICIPATED IN ONE OF THE 24 GROUPS OFFERED. MASTER'S CLINICIANS PROVIDED INDIVIDUAL COUNSELING TO 183 FROM IHH/ACT AND RESIDENTIAL TEAMS.

HOUSING, WORKFORCE DEVELOPMENT & BASIC NEEDS SERVICES -VARIOUS PROGRAMS, SUCH AS RECOVERY HOUSING, AND THE WOONSOCKET SHELTER STABILIZE HOUSING SITUATIONS OF PEOPLE IN A VARIETY OF SITUATIONS. 45 MEN LIVED IN RECOVERY HOUSING AT A STAGE IN THEIR ACTIVE RECOVERY FROM SUBSTANCE USE AND CO-OCCURRING DISORDERS WHEN A SAFE, SOBER LIVING SITUATION WAS NEEDED. THE SHELTER HOUSED 158 PEOPLE (85 ADULTS AND 73 CHILDREN). THE FAMILY SUPPORT CENTER SERVED APPROXIMATELY 3641 HOUSEHOLDS WHICH REPRESENTS 8759 PERSONS. THE PROGRAM OFFERS BASIC NEEDS ASSISTANCE TO LOW INCOME, WOONSOCKET HOUSEHOLDS OFFERING BASIC NEEDS ASSESSMENTS, GUIDED REFERRALS, ADVOCACY AND FINANCIAL ASSISTANCE TO STRENGTHEN FINANCIAL CONDITION. A RANGE OF TRAINING PROGRAMS HELP PEOPLE GET BACK INTO THE WORKFORCE, INCLUDING SKILLS-BUILDING CLASSES, WORK READINESS, JOB SHADOWING AND JOB PLACEMENT AND ACADEMIC ENRICHMENT PROGRAMS. PROJECT LEARN PROVIDED ESL, GED AND BASIC SKILLS CLASSES TO 155 INDIVIDUALS WITH 14 STUDENTS GAINING EMPLOYMENT AND 40 RETAINING EMPLOYMENT. 85 PARTICIPATED IN ESL CLASSES AND 70 PARTICIPATED IN ADULT BASIC EDUCATION CLASSES. 13 RECEIVED CERTIFICATION WITH NORTH STAR DIGITAL LITERACY. THE HARBOUR YOUTH CENTER PROVIDED CAREER ENGAGEMENT OPPORTUNITIES TO 2500 YOUTH AND YOUNG ADULTS WITH VIRTUAL PROGRAMMING AFTER MARCH. 18 PAID PROGRAM PARTICIPANTS GRADUATED WITH POSITIVE EMPLOYMENT AND/OR EDUCATIONAL OUTCOMES. 70 YOUTH PARTICIPANTS UTILIZED WORK READINESS WORKSHOPS AND 40 WIOA ELIGIBLE YOUTH RECEIVED WORKFORCE DEVELOPMENT SERVICES AND CASE MANAGEMENT. CCA PARTNERS WITH OTHER CAP AGENCIES TO PROVIDE RI WORKS PROGRAMMING TO ASSISTED AN AVERAGE OF 153 LOW INCOME CLIENTS FACING BARRIERS TO EMPLOYMENT PER MONTH. MONEY SENSE OFFERED FINANCIAL LITERACY CLASSES TO 18 CASE MANAGERS AND 329 INDIVIDUALS, FACILITATED 41 FINANCIAL LITERACY WORKSHOPS AND INCREASED OUTREACH TO VOLUNTEER INCOME TAX ASSISTANCE PROGRAM RECIPIENTS.

FAMILY WELL-BEING AND PERMANENCY- THESE PROGRAMS PROVIDE FAMILY-CENTERED, TRAUMA-INFORMED CARE INCLUDING OFFICE-BASED, CLINICAL SERVICES FOR CHILDREN AND FAMILIES STRUGGLING WITH BEHAVIORAL HEALTH ISSUES; AND INTENSIVE, COMMUNITY-BASED SERVICES FOR INDIVIDUALS IN ACUTE DISTRESS AND AT HIGH-RISK OF HARM TO SELF OR OTHERS. THE ADULT GENERAL OUTPATIENT PROGRAM SERVED 1726 ACTIVE CLIENTS AND INCREASED SHOW RATES BY 20%. 806 FAMILIES WERE SERVED BY CHILDREN'S OUTPATIENT SERVICES. THE HEALTHY TRANSITIONS TEAM PROVIDES A COMPREHENSIVE ARRAY OF SERVICES TO YOUNG PEOPLE AGED 16-25 WITH A SERIOUS MENTAL HEALTH OR CO-OCCURRING DISORDER, SERVING 82 CLIENTS THIS FISCAL YEAR. THE CHILD WELFARE PROGRAMS COLLABORATE WITH THE DEPARTMENT OF CHILDREN, YOUTH AND FAMILIES TO REDUCE CHILD ABUSE AND NEGLECT. THESE INCLUDE THE FAMILY CARE COMMUNITY PARTNERSHIP, INTENSIVE FAMILY PRESERVATION, FAMILY VISITATION PROGRAMS AND THERAPEUTIC FOSTER CARE. THE AGGREGATE NUMBER OF CHILDREN AND THEIR FAMILIES SERVED WAS 479. THE EARLY INTERVENTION PROGRAM STARTED THE YEAR WITH 297 ELIGIBLE CHILDREN AND RECEIVED 237 NEW REFERRALS IN THE FISCAL YEAR, SERVING ELIGIBLE CHILDREN FROM BIRTH TO 3 YEARS OF AGE TO PROMOTE THEIR GROWTH AND DEVELOPMENT BY ADDRESSING CHILDREN'S DEVELOPMENTAL DELAYS OR FUNCTIONAL SKILLS LEVELS, AND CERTAIN OTHER DIAGNOSED CONDITIONS. OTHER EARLY CHILDHOOD PROGRAMS SUPPORT NEW MOTHERS, PROVIDING HEALTH ASSESSMENTS, MATERNAL DEPRESSION SCREENINGS, AND DEVELOPMENTAL SCREENINGS FOR CHILDREN UNDER THREE AND SHORT TERM VISITS TO IDENTIFY AND GUIDE REFERRALS TO LONGER TERM SERVICES AGREED TO BY THE FAMILY.
