THE MISSION AT ROCKY MOUNTAIN HEALTH CARE SERVICES IS IMPROVING LIVES, OPTIMIZING WELLNESS, AND PROMOTING INDEPENDENCE FOR SENIORS.
We help older adults maintain the highest level of independence possible by offering a coordinated, holistic suite of services to meet their healthcare needs.
THE PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY (PACE) IS A NATIONAL MANAGED CARE PROGRAM OPERATED THROUGH FUNDING FROM MEDICARE, MEDICAID AND OTHER FEDERAL AGENCIES ON A PER MEMBER PER MONTH BASIS. AT ROCKY MOUNTAIN PACE, OUR GOAL IS TO KEEP SENIORS 55 AND OLDER IN OUR COMMUNITY INDEPENDENT AND ACTIVE FOR AS LONG AS POSSIBLE. OUR RESOURCES AT PACE ALLOW US TO PROVIDE COMPREHENSIVE CARE TO MEET ALL HEALTH CARE NEEDS OF OUR PARTICIPANTS, RANGING FROM PRIMARY AND SPECIALTY MEDICAL CARE TO HOME HEALTH CARE, AS WELL AS PROVIDING PHYSICAL THERAPY AND TRANSPORTATION TO MEDICAL AND DENTAL APPOINTMENTS. WE ALSO FOCUS ON SOCIALIZATION AND RECREATION OPPORTUNITIES THROUGH OUR DAY CENTER. IN 2019, THE PACE PROGRAM PROVIDED 8,345 MEMBER MONTHS OF SERVICE TO 721 INDIVIDUAL PARTICIPANTS.
ROCKY MOUNTAIN BRAINCARE PROVIDES DAILY SUPPORT FOR INDIVIDUALS WITH AN ACQUIRED BRAIN INJURY (ABI), INCLUDING TRAUMATIC BRAIN INJURY (TBI). OUR FOCUS AT BRAINCARE IS NOT JUST TO HELP OUR PARTICIPANTS RECOVER, BUT THRIVE AFTER A BRAIN INJURY. THE RESIDENTIAL LIVING PROGRAM, WHICH CONSISTS OF THREE LICENSED ASSISTED LIVING COMMUNITIES as of December 31, 2019, IS THE FOUNDATION OF EVERYTHING WE DO AT BRAINCARE. WE ALSO OFFER A RANGE OF COGNITIVE REHABILITATION SERVICES, AS REACQUIRING LOST SKILLS AND LEARNING NEW ONES ARE THE ULTIMATE GOALS OF BRAIN INJURY RECOVERY. IN 2019, AN AVERAGE OF 20 INDIVIDUAL PARTICIPANTS WERE SERVED THROUGH THIS PROGRAM.
