Organizations Filed Purposes: CIVHC strives to empower individuals, communities, and organizations through collaborative support services and health care information to advance the Triple Aim of better health, better care, and lower costs.

COLORADO ALL PAYER CLAIMS DATABASEIn 2010, the Executive Director of the Colorado Department of Health Care Policy and Financing (HCPF) appointed CIVHC the Administrator of the Colorado All Payer Claims Database (CO APCD). With implementation grant funding from The Colorado Trust and the Colorado Health Foundation, CIVHC began collecting claims from payers and providing data analysis to inform opportunities and advance the Triple Aim in 2012. The CO APCD is now the state's most comprehensive claims data set representing the majority of insured lives in Colorado and including all major commercial payers, Medicaid and Medicare. CIVHC releases CO APCD data in two ways: non-public custom releases, licensed by Change Agents working on specific projects to improve care for Coloradans; and public information on civhc.org designed to foster decision-making at all levels of the health care system, from consumer to state agencies.PUBLIC DATA RELEASESIncreasing access to transparent health care data for all stakeholders is foundational to the original legislative vision of the CO APCD, CIVHC's mission, and to Colorado's ability to make informed decisions that will have lasting benefit to the state. Public analyses and interactive tools, available on civhc.org is one of the methods CIVHC employs to bring transparency to the health care marketplace.NON-PUBLIC DATA RELEASESIn addition to making public information available, CIVHC provides custom data sets and reports to state agencies, organizations and researchers seeking to advance the Triple Aim. Every release of data must benefit Colorado, as mandated by CO APCD regulations.

DELIVERY SYSTEM REDESIGNPalliative Care CIVHC has become a national go-to source for insight into palliative care issues through convening, reporting and analyses. In 2014, CIVHC and taskforce partners from across the state successfully appealed to the Colorado State Board of Health to include a definition of palliative care as part of the Colorado Standards for Hospitals and Health Facilities.Advance Care PlanningAdvance Care Planning allows individuals to share and document their wishes surrounding serious illness, incapacitation, and end of life. Each month, CIVHC currently convenes an Advance Care Planning Work Group where stakeholders discuss issues facing the palliative care community and brainstorm ideas about how to affect meaningful change. CIVHC also hosts the Medical Orders for Scope of Treatment (MOST) form on civhc.org. The MOST form is a 1-page, 2-sided document that consolidates and summarizes patient preferences for key life-sustaining treatments: CPR, medical interventions and artificially administered nutrition. The program was established by legislation (C.R.S. 18.7) in Colorado in 2010.
