(773) 373-1073; 2659 W. 59TH STREET (ADMINISTRATIVE OFFICE RESIDENTIAL SERVICES (773) 918-6200). WE PROVIDE HOUSING AND REHABILITATION SERVICES FOR ADULTS 16 AND OLDER WHO HAVE DISABILITIES. EMPLOYMENT TRAINING AND MONITORING IS ALSO PROVIDED. SIXTY-FOUR CLIENTS ARE SERVED IN THE INTEGRATED CARE FACILITIES FOR DEVELOPMENTAL DISABILITIES (ICF/DD) AND FIFTY-SIX CLIENTS ARE SERVED IN THE EIGHT COMMUNITY INTEGRATED LIVING ARRANGEMENTS (CILAS). THESE ARE CONSIDERED PRIVATE RESIDENCES AND ADDRESSES AND TELEPHONE NUMBERS ARE NOT PUBLISHED.

WE PROVIDE VOCATIONAL TRAINING AND HABILITATION SERVICES, JOB PLACEMENT, JANITORIAL SERVICES, INTAKE / OUTREACH, VOCATIONAL EVALUATION, DEVELOPMENTAL TRAINING, WORK SERVICES, WORK ADJUSTMENT, AND SUPPORTIVE EMPLOYMENT SERVICES.

CHILD WELFARE SERVICES: FOSTER FAMILY CARE AND ADOPTION SERVICES OPERATE OUT OF TWO PROGRAM SITES: 7640 S. VINCENNES AVENUE AND 7718 SOUTH PHILLIPS AVENUE. TRADITIONAL FOSTER CARE SERVICES (773) 602-2660, FOSTER CARE EMERGENCY SHELTER SERVICES (773)356-6972. WE SERVE WARDS OF THE STATE, THEIR SIBLINGS, BIOLOGICAL AND FOSTER PARENTS THROUGH LICENSING OF HOMES, PLACEMENT, AND PERMANENCY SUPPORT, CASE MANAGEMENT, POST PERMANENCY SUPPORT, AND CHILD PROTECTION. THE PROGRAM SERVED 211 CHILDREN THIS YEAR.
