RESEARCH/ACADEMIC GRANT PROJECTS FOR THE PERIOD 7/1/2018 - 6/30/2019 INCLUDED STATE, FEDERAL AND PRIVATE NON-PROFIT RESEARCH AWARDS FOR THE FOLLOWING PROJECTS: (I) IMPROVING THE HEALTH OF SEXUAL AND GENDER MINORITY PATIENTS THROUGH CULTURAL COMPETENCE AND SKILL TRAINING OF CHC STAFF; (II) COMPARATIVE EFFECTIVENESS OF COMMUNITY HEALTH WORKERS IN CHICAGO AND NEW YORK CITY TO PREVENT DESTABILIZATION AND UNPLANNED HOSPITALIZATIONS OF ADULTS WITH MULTIPLE CHRONIC DISEASES; (III) CONNECTIONS FOR THE HOMELESS AND REFUGEES TO MEDICAL HOMES; (IV) CHICAGO CARES NETWORK OF CDC RECOGNIZED LIFESTYLE CHANGE PROGRAMS TO REDUCE DIABETES DISPARITIES AND PROMOTE HEALTH EQUITY IN UNDER SERVED COMMUNITIES CONTROL DIABETES; AND (V) LINKING INTER-SPECIALTY NEWBORN AND CONTRACEPTION CARE AS A NOVEL APPROACH TO POSTPARTUM CONTRACEPTION PROVISION AT THE WELL-BABY VISIT.
PROFESSIONAL SERVICES RELATE TO MULTIPLE CONSULTING CONTRACTS TO ASSIST WITH: (I) CEO/CMO ROUND TABLES, TRAININGS AND WEBINARS; (II) RESEARCH READINESS & INFRASTRUCTURE DEVELOPMENT; (III) USING TECHNOLOGY TO PREVENT CHILDHOOD OBESITY IN LOW-INCOME FAMILIES & COMMUNITIES; (IV) DEVELOPMENT AND TESTING FOR VALIDITY & RELIABILITY OF ELECTRONIC CLINICAL QUALITY MEASURES (ECQMS) FOR ALL AND POSTPARTUM WOMEN; (IV) PROVIDING COACHING, SUPPORT, AND SHARED LESSONS FOR CHILDHOOD OBESITY MANAGEMENT THROUGH IMPLEMENTATION OF MIND, EXERCISE AND NUTRITION PROGRAMS; (V) MEDICATION OPTIMIZATION IN PRIMARY CARE; (VI) BUILDING CAPACITY IN FQHCS TO IMPROVE POSTPARTUM DIABETES SCREENING; AND (VII) PROMOTING IMPROVED INTERVENTION & CLINICAL OUTCOMES IN DIABETES CARE.
