THE CENTER IS ORGANIZED TO PROMOTE THE COORDINATION, RESEARCH, EVALUATION, AND POLICY DEVELOPMENT OF HOUSING AND HEALTH PROGRAMS THAT SERVE VULNERABLE POPULATIONS IN THE CHICAGO METROPOLITAN AREA.
THE CENTER CONTINUES TO DEMONSTRATE SIGNIFICANT DECREASES IN UNNECESSARY HEALTH CARE USE AND REDUCED COST THROUGH ITS BETTER HEALTH THROUGH HOUSING PROJECT. THE CENTER CONTINUES TO OFFER PERMANENT SUPPORTIVE HOUSING AND WRAPAROUND CASE MANAGEMENT SERVICES FOR ITS CLIENTS. OVER 900 PAYMENTS PER MONTH ARE PROVIDED TO PRIVATE MARKET INDIVIDUAL LANDLORDS TO PROVIDE HOUSING FOR ITS CLIENT, INCLUDING OTHER DIRECT CLIENT SUPPORT SUCH AS UTILITY ASSISTANCE, FOOD, AND TRANSPORTATION.
CHH CONTINUED ITS WORK AS THE ADMINISTRATOR OF THE INNOVATIVE FLEXIBLE HOUSING POOL. IN PARTNERSHIP WITH COOK COUNTY HEALTH, UNIVERSITY OF ILLINOIS HEALTH, ADVOCATE AURORA HEALTH AND OTHER HOSPITAL SYSTEMS, 109 PEOPLE WHO EXPERIENCED HOMELESSNESS NOW HAVE SAFE HOMES OR ARE IN THE PROCESS OF GETTING HOMES. THANKS TO A $5 MILLION INVESTMENT FROM THE CITY OF CHICAGO, THE FLEXIBLE HOUSING POOL PROGRAM WAS ABLE TO EXPAND AND FOCUS ON A SECURING PERMANENT HOUSING FOR YOUNG ADULTS AGES 18 TO 24 YEARS OLD, SOME OF WHOM HAVE CHILDREN IN THEIR HOUSEHOLD. THESE YOUNG FAMILIES WHO PREVIOUSLY EXPERIENCED HOUSING INSECURITY NOW NOT ONLY HAVE FURNISHED HOMES, BUT THEY ALSO HAVE ACCESS TO RESOURCES INCLUDING DIAPERS AND PARENTING CLASSES.
CHH COORDINATED THE WORK OF OVER 105 SKILLED ASSESSORS FROM MORE THAN 50 AGENCIES THROUGHOUT CHICAGO TO ENSURE THAT ALL PEOPLE EXPERIENCING HOMELESSNESS ARE ASSESSED FOR HOUSING RESOURCES IN THE COORDINATED ENTRY SYSTEM AND HAVE MORE EQUITABLE ACCESS TO THIS HUMAN RIGHT.
