Community Nursing Service of DuPage County (CNS) (dba Northwestern Medicine Home Health & Hospice) provides comprehensive, full-service home health and hospice services to patients in a home setting, regardless of the patient's ability to pay. Effective June 1, 2019, the business operations of CNS were sold to JourneyCare, a 501(c)3 organization located in the same geographic area. CNS entered into a winding-down stage. Prior to June 1, 2019, CNS offered the following services: HOME HEALTH Home Health services are available to individuals considered homebound. Our home health team is dedicated to helping patients improve their quality of life after leaving the hospital or any other time assessment, education and skilled intermittent care are required. Services offered include: Skilled nursing, Home health infusion, Home health aides, Medical social workers, Physical therapy, Occupational therapy, Speech therapy. HOSPICE Hospice is a philosophy of care that accepts death as the final stage of life. The goal of hospice is to enable patients to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality, surrounded by their loved ones. Hospice is designed to offer comfort and care, helping relieve issues that occur in the final phases of an end stage disease process, and is designed for people whose illnesses no longer respond to curative treatment but focuses on treating symptoms while increasing quality of life. Hospice care includes not only support for the patient, but also the family and loved ones. Northwestern Medicine Home Health & Hospice provides a caring hospice team that works with you to develop a care plan with goals to: improve quality of life; relieve pain, control symptoms and offer comfort and dignity; and, provide physical, emotional, social and spiritual support. As experienced providers of end-of-life care, the staff and volunteers at Northwestern Medicine Home Health & Hospice recognize that grief is a powerful force-one that can leave you feeling alone, uncertain, frightened and sorrowful. Our trained staff provides support during this difficult period.
