Organizations Filed Purposes:
TO IMPROVE THE LIVES OF AT-RISK YOUTH INVOLVED IN CHILD WELFARE AND JUVENILE JUSTICE.
THE ORGANIZATION OPERATES A PROGRAM DESIGNED FOR YOUTH THAT ARE DUALLY INVOLVED IN BOTH THE CHILD WELFARE AND JUVENILE JUSTICE SYSTEMS IN COOK AND LAKE COUNTIES. YOUTH PARTICIPATE IN A WRAPAROUND FACILITATION MODEL INTENDED TO DEVELOP A SUPPORT SYSTEM FOR THE YOUTH AND ADDRESS UNMET NEEDS OF THE YOUTH AND FAMILY. THE YOUTH'S TEAM DEVELOPS AN ACTION PLAN WITH THE GOALS OF SUPPORTING YOUTH WITHIN THEIR OWN COMMUNITY, PREVENTING FUTURE CRIMINAL BEHAVIOR, AND HELPING YOUTH TRANSITION TO ADULTHOOD. THE PROGRAM HAS SEEN A REDUCTION IN DETAINMENT AND CONGREGATE CARE LIVING ARRANGEMENTS, IN COMPARISON TO BASELINE DATA PROVIDED BY ITS FUNDER AND VIA AN EXTERNAL EVALUATION.
