LOCATED IN THE ALTGELD-MURRAY HOMES PUBLIC HOUSING DEVELOPMENT , TCA HEALTH IS A MEDICAL HOME FOR THOUSANDS OF RESIDENTS ON THE FAR SOUTH-SIDE OF CHICAGO AND ITS SURROUNDING SUBURBS. ANNUALLY, TCA HEALTH SERVES NEARLY 7,000 PATIENTS. PATIENTS ARE GIVEN AFFORDABLE ACCESS TO CULTURALLY AND LINGUISTICALLY APPROPRIATE HEALTH CARE OF HIGH QUALITY. AT THE MAIN LOCATION, CLINICAL SERVICES INCLUDE INTERNAL/FAMILY MEDICINE, DENTAL, PEDIATRICS, OBSTETRICS & GYNECOLOGY, BEHAVIORAL HEALTH, OPTOMETRY, PHARMACY, LAB AND ULTRASOUND. TCA HEALTH'S REACH NOW EXTENDS BEYOND THE ALTGELD-MURRAY HOMES PUBLIC HOUSING DEVELOPMENT INTO OTHER SOUTH SIDE COMMUNITIES THROUGH ITS RECENTLY FUNDED MOBILE VAN HEALTH CLINIC, AND THE CARVER MILITARY ACADEMY SCHOOL BASED HEALTH CENTER.
THE SPECIAL SUPPLEMENTAL NUTRITION PROGRAM FOR WOMEN, INFANTS, AND CHILDREN (WIC) PROVIDES FEDERAL GRANTS TO STATES FOR SUPPLEMENTAL FOODS, HEALTH CARE REFERRALS, AND NUTRITION EDUCATION FOR LOW-INCOME PREGNANT, BREASTFEEDING, AND NON-BREASTFEEDING POSTPARTUM WOMEN, AND TO INFANTS AND CHILDREN UP TO AGE FIVE WHO ARE FOUND TO BE AT NUTRITIONAL RISK. WIC HELPS PARENTS TO FEED THEIR CHILDREN PROPERLY DURING CRITICAL PERIODS OF GROWTH AND DEVELOPMENT. THE COMBINATION OF NUTRITION EDUCATION, NUTRITIOUS FOODS, AND YEARLY HEALTH CARE VISITS STRENGTHENS FAMILIES LONG AFTER THEIR WIC ELIGIBILITY HAS ENDED.
THE HEALTH AND WELLNESS COLLABORATIVE PROMOTES HEALTHY LIFESTYLE OPTIONS AND ACCESS TO WELLNESS ACTIVITIES AMONG THE CHILDREN AND FAMILIES OF CHICAGO'S FAR SOUTH-SIDE AND SURROUNDING SUBURBS. THE HEALTH AND WELLNESS COLLABORATIVE PROVIDES COMMUNITY RESIDENTS WITH THE OPPORTUNITY TO PARTICIPATE IN FITNESS PROGRAMS, COMMUNITY GARDENING AND UTILIZE FOOD ACCESS AND NUTRITION SERVICES TO PROMOTE A HEALTHY LIFE AND COMMUNITY. ASIDE FROM OUR MANY NUTRITION, FITNESS AND COMMUNITY GARDENING PROGRAMS, THE COLLABORATIVE HAS ALSO TAKEN THE LEAD TO ADDRESS FOOD INSECURITIES WITHIN SEVERAL FAR SOUTH SIDE COMMUNITIES WHERE HEALTH DISPARITIES CONTINUE TO PLAGUE FAMILIES. THE COLLABORATIVE PARTNERS WITH LOCAL ORGANIZATIONS AND SCHOOLS TO PROVIDE 6 TO 8 WEEKS EVIDENCE BASE HEALTH AND WELLNESS SERVICES TO CHILDREN AND ADULTS GROUPS ON SITE.
