Organizations Filed Purposes: TO PROVIDE HOUSING SOLUTIONS, SUPPORT SERVICES AND COUNSELING FOR SENIORS, DISABLED AND OTHERS IN NEED, PROMOTING PHYSICAL AND MENTAL WELLBEING AND ALLEVIATING ISOLATION TO ENABLE THEM TO LIVE WITH DIGNITY AND INDEPENDENCE.

SENIOR SUPPORT AND GERIATRIC COUNSELING OFFERS SUPPORT FOR THE FRAIL ELDERLY, PERSONS WITH DISABILITIES AND CARE PARTNERS COPING WITH CHALLENGES RELATED TO AGING. CASE MANAGERS PROVIDE FREE IN-HOME ASSESSMENTS, ONGOING CASE MANAGEMENT SERVICES AND ACCESS TO COMMUNITY RESOURCES TO STABILIZE "AT RISK" RESIDENTS. STAFF IS SUPPORTED BY OVER 300 VOLUNTEERS PROVIDING SERVICES TO ADDRESS ISOLATION AND HELP RESIDENTS REMAIN INDEPENDENT IN THEIR HOMES AND ACCESS AFFORDABLE HOUSING OPTIONS. WEEKLY FRIENDLY VISITS, DAILY TELEPHONE REASSURANCE CALLS, HELP WITH SHOPPING AND LIGHT HOME MAINTENANCE SERVICES ARE PROVIDED WITHOUT COST TO RESIDENTS.LICENSED COUNSELORS PROVIDE INDIVIDUAL COUNSELING SESSIONS TO SENIORS AND ADULTS OF ALL AGES TO ALLEVIATE DEPRESSION AND HELP WITH A WIDE RANGE OF DIFFICULTIES. SENIORS AND FAMILY MEMBERS RECEIVE MEDICARE AND GERIATRIC COUNSELING SUPPORT TO NAVIGATE SHORT AND LONG TERM HEALTH CARE NEEDS AND EFFECTIVE REFERRALS TO AREA RESOURCES. OVER 11,000 HOURS OF HOUSEKEEPING SERVICES WERE PROVIDED TO SENIORS THROUGH THE CENTER'S CONTRACTED "CHORE AIDE" WORKERS AND SENIOR COMPANIONS DELIVERED 60,000 OF SERVICES WITHOUT COST TO OLDER ADULTS IN SURBURBAN COOK COUNTY.THE OVERALL PROGRAM GOAL IS TO HELP OLDER ADULTS REMAIN AS INDEPENDENT AS POSSIBLE WITH ACTIVITIES TO REDUCE ISOLATION AND ADDRESS CHALLENGES RELATED TO AGING. ADULTS SEPARATED FROM FAMILY AND WISH TO REMAIN IN THEIR HOME AND/OR ARE UNABLE TO AFFORD THE RISING COSTS OF IN-HOME HEALTHCARE AND ASSISTED LIVING ARRANGEMENTS. CASE MANAGERS OFFER UNBIASED SOLUTIONS FOR RESIDENTS AND ADVOCATE FOR THE NEEDS OF OLDER RESIDENTS AS THEY NAVIGATE THE HEALTHCARE SYSTEM, OBTAIN SUPPORTIVE SERVICES, AND TRANSITION INTO ASSISTED LIVING FACILITIES.

OVER 27,000 SERVICES WERE PROVIDED IN THE AGENCY'S SENIOR SUPPORT, HOUSING SOLUTIONS AND COMMUNITY SERVICES PROGRAMS LAST FISCAL YEAR. A MONDAY THROUGH FRIDAY DAILY NUTRITION PROGRAM FOR ADULTS OVER 60 SERVED OVER 5,000 MEALS AND TWO NEW MEMORY CAFES PROVIDED A SAFE ENVIRONMENT FOR PERSONS IMPACTED BY MEMORY LOSS AND FAMILY CARE PARTNERS CENTERED ON ART, MUSIC, AND DANCE THERAPY. HOUSING COUNSELING AND ASSISTANCE PROGRAMS PROVIDED CLIENTS WITH POTENTIAL HOUSING OPTIONS INCLUDING TRANSITIONAL HOUSING FOR THE HOMELESS.

A TOTAL OF 49 PERSONS (ADULTS AND CHILDREN); 21 HOUSEHOLDS FOR RAPID RE-HOUSING WERE ASSITED THROUGH THE HUD PROGRAM. A HOME SHARING PROGRAM PROVIDES ISOLATED HOMEOWNERS WITH ADDITIONAL INCOME AND AFFORDABLE HOUSING TO RESPONSIBLE ADULTS.ASSIST WITH ACCESSING HOUSING CHOICE VOUCHER PLACEMENT, SENIOR HOUSING APPLICATIONS, LANDLORD AND REALTOR NEGOTIATIONS, REFERRALS TO LOW-COST APARTMENTS, SHELTERS, AND SINGLE ROOM OCCUPANCY SITES.FACILITATE REFERRALS TO MENTAL HEALTH AND OTHER SUPPORTIVE SERVICES TO HOMELESS FAMILIES AND INDIVIDUAL TO BUILD LIFE SKILLS TOWARD SELF-SUFFICIENCY.HOMELESSNESS PREVENTION EXPENSES ASSOCIATED WITH THE FINANCIAL ASSISTANCE TO AVOID EVICTION OR FORECLOSURE OR TO PREVENT UTILITY DISCONNECTION. THE PROGRAM ALSO INCLUDES CASE MANAGEMENT AND LEGAL, FINANCIAL AND/OR EMPLOYMENT COUNSELING THROUGHOUT ALL PROGRAMS.THE ORGANIZATION'S PROFESSIONAL VOLUNTEERS PROVIDED 9,803 HOURS OF SERVICES FOR CLIENTS THAT INCLUDED WILLS, LEGAL, FINANCIAL, TAX PREPARATION, MEDICARE EMPLOYMENT AND COUNSELING.
