SUPPORTIVE HOUSING PROGRAM: THE PERMANENT SUPPORTIVE HOUSING PROGRAM PROVIDES LONG-TERM RENTAL ASSISTANCE AND SUPPORTIVE SERVICES TO INDIVIDUALS WHO ARE HOMELESS AND HAVE A DISABILITY. IN 2019, 408 INDIVIDUALS WERE SERVED THROUGH THIS PROGRAM, INCLUDING 101 INDIVIDUALS WHO WERE SERVED BY PARTNER AGENCIES.
RAPID RE-HOUSING: RAPID RE-HOUSING MINIMIZES THE TRAUMA OF HOMELESSNESS BY DIVERTING INDIVIDUALS AND FAMILIES FROM EMERGENCY SHELTER AND RE-HOUSING THEM AS QUICK AS POSSIBLE. CLIENTS RECEIVED SHORT-TERM (UP TO 3 MONTHS) TO MEDIUM-TERM (4-24 MONTHS) RENTAL SUBSIDIES COUPLED WITH CASE MANAGEMENT, WRAP-AROUND SUPPORTIVE SERVICES, AND ONGOING NON-FINANCIAL STABILITY SERVICES AFTER THE RENTAL ASSISTANCE ENDS TO ENSURE LONG-TERM HOUSING STABILITY. CRITICAL TO THE PROGRAM'S SUCCESS IS OUR PARTNERSHIPS WITH MORE THAN 20 PRIVATE LANDLORDS. ALL OF THE HOUSING FOR RRH IS MARKET-RATE HOUSING PROVIDED THROUGH PRIVATE LANDLORDS. IN 2019, 59 PERSONS RECEIVED ASSISTANCE.
COORDINATED ENTRY (CE): A COMMUNITY-WIDE SYSTEM TO STANDARDIZE AND EXPEDITE THE PROCESS BY WHICH PEOPLE EXPERIENCING HOMELESSNESS OR AT RISK OF HOMELESSNESS ACCESS HOUSING AND HOMELESS RESOURCES. THROUGH CE, PEOPLE EXPERIENCING HOMELESSNESS ARE MATCHED TO AVAILABLE SERVICES AND HOUSING BASED ON THEIR PREFERENCES AND LEVEL OF NEED. COORDINATED ENTRY IN SUBURBAN COOK COUNTY IS LED BY HOUSING FORWARD WHO WILL OVERSEE A TEAM OF EIGHT IMPLEMENTING PARTNERS WHO WERE SELECTED THROUGH A COMPETITIVE RFP PROCESS BASED UPON THEIR HISTORY OF DELIVERING SERVICES TO THE REGIONS' AT-RISK AND HOMELESS POPULATIONS, THEIR COMMITMENT TO HOUSING FIRST AND HARM REDUCTION PRACTICES, GEOGRAPHIC LOCATION AND A WILLINGNESS TO EMBRACE PRINCIPLES OF COORDINATED ENTRY.
