AGEOPTIONS IS A NOT-FOR-PROFIT CORPORATION DESIGNATED BY THE ILLINOIS DEPARTMENT ON AGING AS THE AREA AGENCY ON AGING FOR SUBURBAN COOK COUNTY. AS A CATALYST FOR CONNECTING OLDER PERSONS AND CAREGIVERS TO RESOURCES AND SERVICE OPTIONS, WE ARE NATIONALLY RECOGNIZED FOR OUR INNOVATIVE PROGRAMMING, NETWORKING WITH COMMUNITY BASED SENIOR SERVICE AGENCIES, AND ADVOCACY EFFORTS ON BEHALF OF OLDER PERSONS. OVER THE PAST 46 YEARS, AGEOPTIONS HAS ESTABLISHED A REPUTATION FOR MEETING THE NEEDS, WANTS AND EXPECTATION OF THE SENIOR POPULATION IN SUBURBAN COOK COUNTY. OUR PURPOSE IS TO CONNECT ADULTS, AGED 60 AND OVER, WITH RESOURCES AND OPTIONS FOR CARE SO THAT THEY CAN HAVE A FULL RANGE OF CHOICES AND LIVE THEIR LIVES TO THE FULLEST. OUR ROLE IS TO PLAN, FUND, COORDINATE AND ADVOCATE FOR THIS SPECIAL GROUP AND THEIR FAMILY CAREGIVERS. WE PRIDE OURSELVES IN ADVOCATING FOR SENIORS IN THE AREAS SUCH AS ELDER JUSTICE, CAREGIVING, NUTRITION, MEDICARE, EMPLOYMENT AND ACCESS TO BENEFITS. OUR PRIMARY SERVICE AREA, SUBURBAN COOK COUNTY, INCLUDES 130 COMMUNITIES WITHIN 30 TOWNSHIPS SURROUNDING CHICAGO - HOME TO MORE THAN 2.5 MILLION RESIDENTS AND 560,901 OLDER ADULTS.THE OLDER AMERICANS ACT TITLE III SOCIAL, NUTRITION, CAREGIVER, AND HEALTH PROMOTION/DISEASE PREVENTION SERVICES ARE CORE SERVICES PROVIDED IN ALL 30 TOWNSHIPS OF SUBURBAN COOK COUNTY. IN FY 2019, 172,077 OLDER ADULTS AND CAREGIVERS WERE SERVED THROUGH THE WIDE RANGE OF TITLE III AND OTHER RELATED PROGRAMS INCLUDING HOME DELIVERED AND CONGREGATE DINING, NURSING HOME OMBUDSMAN, LEGAL SERVICE, AND THE OTHER SOCIAL SERVICES.

STATE/GRF - GENERAL REVENUE FUNDS FOR THE PURPOSE OF ABUSE INTERVENTION, TRANSPORTATION, OUT-REACH AND HOME DELIVERED MEALS.

THE SMP (SENIOR MEDICARE PATROL) PROGRAM IS A NATIONAL PROGRAM THAT EMPOWERS MEDICARE AND MEDICAID BENEFICIARIES TO PREVENT, DETECT, AND REPORT HEALTH CARE FRAUD. AGEOPTIONS LEADS THE ILLINOIS SMP PROGRAM WITH FUNDING FROM THE ADMINISTRATION FOR COMMUNITY LIVING, U.S. DEPARTMENT OF HEALTH AND HUMAN SERVICES. AGEOPTIONS IS ABLE TO REACH ALL AREAS OF ILLINOIS WITH THE SMP MESSAGE BY PARTNERING WITH THE 13 ILLINOIS AREA AGENCIES ON AGING, WHITE CRANE WELLNESS CENTER, CATHOLIC CHARITIES AND THE COALITION OF LIMITED ENGLISH SPEAKING ELDERLY. TRAINED VOLUNTEERS AND STAFF GIVE COMMUNITY PRESENTATIONS, ATTEND OUTREACH EVENTS SUCH AS HEALTH AND SENIOR FAIRS, AND PROVIDES INDIVIDUAL COUNSELING TO MEDICARE AND MEDICAID BENEFICIARIES WHO MIGHT HAVE POTENTIAL MEDICARE FRAUD. OUR SMP MESSAGES IS AS FOLLOWS:PROTECT - PROTECT YOURSELF FROM MEDICARE ERRORS, FRAUD, OR ABUSE BY NEVER GIVING YOUR MEDICARE NUMBER TO STRANGERS WHO CALL OR VISIT YOUR HOME.DETECT - DETECT POTENTIAL ERRORS, FRAUD, OR ABUSE BY READING YOUR MEDICARE SUMMARY NOTICE OR EXPLANATION OF BENEFITS FROM YOUR INSURANCE COMPANY.REPORT - IF YOU SUSPECT THAT YOU HAVE BEEN A TARGET OF ERRORS, FRAUD, OR ABUSE, REPORT IT TO THE ILLINOIS SMP AT AGEOPTIONS. IN FY 2019, SMP REACHED 34,875 PEOPLE THROUGH 720 GROUP OUTREACH AND EDUCATION EVENTS AND COUNSELED 4,809 PEOPLE INDIVIDUALLY ON HEALTH CARE FRAUD TOPICS STATEWIDE. WE ACCOMPLISHED THIS WITH 102 TEAM MEMBERS (VOLUNTEERS AND STAFF AT OUR PARTNER AGENCIES) WHO CONTRIBUTED 8,187 HOURS TO THE PROGRAM.
