Easterseals Michigan works to change the way the world defines and views disabilities by making profound, positive differences in people's lives every day. For 100 years, we have been serving Michigan residents. During this past year we have served and supported more than 13,000 children and adults with disabilities and their families. As the world grew, so did Easterseals Michigan. Today our mission serves people of all ages with many different disabilities. Our specialties include addressing behavioral health and/or substance use conditions; assessing and providing trauma services; providing early intervention, autism and medical rehabilitation services.

Adult Services - ESM provides community and office based therapeutic and rehabilitative evidenced based services and supports to adult individuals who have been diagnosed with a mental health, substance use and/or co-occurring condition(s). ESM served 9,367 adults in the past year. Program locations include Older Adults in Kent County, Southfield Adults, Pontiac Adults and Macomb Centerline, Agrability for farmers. ESM operates a number of programs with a diverse funding structure in each of these locations. Outcomes: Experience of care FY20 81.9% satisfaction; National Outcome Measures experience of care: 84.7%Timliness Outcomes: Request to First Service: 79.54%Intake to ongoing services within 14 days: 99.9%Continuity of Care post discharge from hospital: 97.14%Recidivism: % of re-admits to psych hospitalization within 30 days: 10.67%.
Family Services - ESM provides evidence based community and office based therapeutic services and supports to children and their families from birth through 18 years of age diagnosed with a mental health, substance use and/or co-occurring condition(s). Services include Applied Behavioral Analysis for children on the Autism Spectrum. ESM served 4,002 children and families with serious emotional disorders, mental illness, trauma and autism in the last year. Program locations include Oakland County - Walled Lake, Waterford, Southfield and Auburn Hills Family Services, Kent County Homebased/case management, Genesee County and Macomb Centerline. ESM operates a number of programs with a diverse funding structure in each of these locations. Outcomes for FY20: Experience of care FY20 81.9% overall; National Outcome Measures of care: 92.5%Timeliness Outcomes: Request to First Service within 14 days (benchmark = 95%): 84.15%Intake to ongoing service within 14 days (benchmark = 95%): 99.25% Continuity of Care post discharge from hospital (benchmark = 95%): 98%Recidivism: % of re-admits to psych hospitalization within 30 days (benchmark = 15%): 10.57%.
Early Intervention, Children's Therapy, Medical Rehab Services, Autism, and Trauma Assessments and Planning -ESM provides Early Intervention, diagnostic, rehabilitative therapy and recreational services to children of all ages. These services include the P.L.A.Y. Project, AAEC - Austism diagnostics, Neurodevelopment Trauma Assessments, Maternal Infant Health Program, Home Visiting Program, Speech/Language Therapy, Occupational Therapy and Miracle League of Michigan. ESM served 922 children and families in the last year. Program locations include Kent County, Waterford, Flint, Auburn Hills, Macomb - Centerline and Southfield. Programs include: Miracle League South Oakland and North Oakland); BC Network/BCBS Autism Assessment Evaluation Center in Auburn Hills, and Medical Rehabilitation Services in West Michigan and Oakland County.
