SUPPORT COMMUNITY-BASED, STAKEHOLDER DRIVEN TRAINING FOR CARE TEAMS

CONDUCTED TRAINING PROGRAMS IN THE AREA OF PATIENT SELF-MANAGEMENT AND COMPLEX CARE MANAGEMENT FOR PRIMARY CARE TEAMS (NURSES, CARE MANAGERS, CARE COORDINATORS, SOCIAL WORKERS AND OTHERS) TO IMPROVE CLINICAL, RESOURCE AND PATIENT EXPERIENCE RESULTS. TRAINING ASSISTED TEAMS IN CARING FOR PATIENTS WITH MEDICARE, MEDICAID AND EMPLOYER-BASED INSURANCE AS WELL AS THE UNINSURED. PROGRAMS WERE OFFERED THROUGHOUT THE STATE OF MICHIGAN TO MEMBER AND NON-MEMBER ORGANIZATIONS ALIKE. 301 CLINICAL AND NON-CLINICAL STAFF PARTICIPATED.

PROVIDED A VARIETY OF TRAININGS FOR MULTIDISCIPLINARY TEAMS OF HEALTH CARE PROFESSIONALS (PHYSICIANS, NURSE PRACTITIONERS, PHYSICIAN ASSISTANTS, CARE MANAGERS, NURSES AND SOCIAL WORKERS) IN PRIMARY CARE PRACTICES TO MANAGE PATIENTS WITH CHRONIC PAIN AND SUBSTANCE USE DISORDERS. THESE TRAININGS INCLUDED:1. DEVELOPED AND CONDUCTED TRAINING THROUGHOUT THE STATE TO IMPROVE THE CARE OF CHRONIC PAIN PATIENTS IN PRIMARY CARE. TRAINING WAS DIRECTED AT TEAMS CARING FOR PATIENTS WITH MEDICARE, MEDICAID, AND COMMERCIAL INSURANCE AS WELL AS THE UNINSURED. 287 PHYSICIANS, NURSE PRACTITIONERS, PHYSICIAN ASSISTANTS, CARE MANAGERS, NURSES AND SOCIAL WORKERS PARTICIPATED.2. SBIRT (SCREENING, BRIEF INTERVENTION AND REFERRAL TO TREATMENT) HEALTH CARE APPROACH FOR EARLY SCREENING, IDENTIFICATION, INTERVENTION, AND TREATMENT TO PEOPLE WITH SUBSTANCE USE DISORDERS AND THOSE AT RISK OF DEVELOPING THESE DISORDERS. 22 NURSES AND SOCIAL WORKERS COMPLETED THE TRAINING.3. MEDICATION ASSISTED TREATMENT TRAININGS TO IMPROVE THE TREATMENT OF PATIENTS WITH SUBSTANCE USE DISORDER. PHYSICIANS AND ADVANCED PRACTICE PROVIDERS (APPS) ATTENDED A 1-DAY SESSION WHICH POSITIONS PHYSICIANS AND APPS TO RECEIVE THE LEARNING REQUIREMENTS NECESSARY TO OBTAIN THE PRESCRIBING WAIVER. 88 PHYSICIANS AND ADVANCED PRACTICE PROVIDERS PARTICIPATED.

CONVENED MULTI COLLABORATIVE STAKEHOLDERS AND LED PLAN DEVELOPMENT TO CREATE AND SUSTAIN A LOCAL COMMUNITY INFORMATION EXCHANGE (CIE) WHEREBY HEALTH CARE PROVIDERS AND COMMUNITY-BASED ORGANIZATIONS CAN COMMUNICATE EFFECTIVELY TO IMPROVE ACCESS TO AND COORDINATION OF SERVICES AND MEET PATIENT/CLIENT NEEDS AND BRINGS TOGETHER COMMUNITY MEMBERS, COMMUNITY SERVICE PROVIDERS, AND HEALTH SYSTEMS TO CREATE A LONG-TERM PARTNERSHIP TO BETTER INTEGRATE HEALTH AND SOCIAL CARE NEEDS WITH A KEEN FOCUS ON CHILDREN AND SENIORS. THE IMPACT OF THIS WORK ADDRESSES PATIENTS WITH MEDICARE, MEDICAID, COMMERCIAL AND THOSE WITHOUT INSURANCE. TARGET AREA IS KENT COUNTY. PARTNERS INCLUDE: HEALTH SYSTEMS, PHYSICIAN ORGANIZATIONS, PUBLIC HEALTH AND COMMUNITY AGENCIES, NAVIGATION HUB.
