SWOPE HEALTH SERVICES (SHS) IMPROVES THE HEALTH AND WELLNESS OF THE COMMUNITY BY DELIVERING ACCESSIBLE, QUALITY, COMPREHENSIVE PATIENT CARE.
SWOPE HEALTH IS A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) SERVING KANSAS CITY AND THE SURROUNDING BI-STATE COMMUNITY, AND IS ACCREDITED BY THE JOINT COMMISSION AND CARF (COMMISSION ON ACCREDITATION OF REHABILITATION FACILITIES). SWOPE HEALTH OPERATES A FULL-SERVICE CLINIC IN KANSAS CITY, MISSOURI, PLUS MISSOURI SATELLITE CLINICS IN BELTON, EAST KC, INDEPENDENCE, HICKMAN MILLS, NORTHLAND AND MAPLE WOODS AND TWO CLINICS IN WYANDOTTE COUNTY, KANSAS. IN 2019, MORE THAN HALF OF SWOPE HEALTH'S PATIENTS ARE AFRICAN AMERICAN, NEARLY A QUARTER ARE WHITE AND MOST OF THE REMAINING PATIENTS ARE HISPANIC/LATINX OR AMERICAN INDIAN/ALASKA NATIVE. WE ARE EXPERIENCING GROWTH IN OUR HISPANIC/LATINX PATIENT POPULATIONS AT MOST LOCATIONS, CONSISTENT WITH RECENT NATIONAL CENSUS DATA.WE HAVE ACHIEVED LEVEL 3 PRIMARY CARE MEDICAL HOME (PCMH) RECOGNITION BY THE NATIONAL COMMITTEE FOR QUALITY ASSURANCE (NCQA). USING ELECTRONIC MEDICAL RECORDS TECHNOLOGY, OUR PROVIDERS HAVE MADE SIGNIFICANT PROGRESS IN MEETING MEANINGFUL USE CRITERIA ESTABLISHED TO DEMONSTRATE QUALITY OF SERVICES PROVIDED TO OUR PATIENTS.SOME EXAMPLES OF OUR ACCOMPLISHMENTS:FREE AND LOW-COST MEDICATIONS: SWOPE HEALTH SECURES MORE THAN $34,000 ANNUALLY IN FREE MEDICATIONS FOR ITS PATIENTS THROUGH VARIOUS PHARMACEUTICAL PATIENT ASSISTANCE PROGRAMS, INCLUDING PFIZER'S "SHARING THE CARE PROGRAM." THIS IS CRITICAL IN MAINTAINING THE HEALTH OF THE UNINSURED PATIENTS AND ASSISTED WITH PURCHASING OF PRESCRIBED MEDICATIONS FOR SWOPE HEALTH PATIENTS WHO ARE WOEFULLY UNDERINSURED. SWOPE HEALTH ALSO PARTICIPATES IN THE HEALTH RESOURCES & SERVICES ADMINISTRATION'S (HRSA) 340B DRUG PRICING PROGRAM OFFERING LOW-COST PRESCRIPTION MEDICATIONS TO OUR UNINSURED AND HOMELESS PATIENTS. MATERNAL AND CHILDREN'S SERVICES: PREGNANT WOMEN IN THE SWOPE HEALTH SERVICE AREA OFTEN DO NOT RECEIVE EARLY AND CONSISTENT PRENATAL CARE. AS A RESULT, THEY ARE MORE LIKELY THAN OTHER PREGNANT WOMEN TO EXPERIENCE INFANT MORTALITY OR DELIVER PREMATURE AND/OR LOW BIRTHWEIGHT INFANTS WITH DEVELOPMENTAL DELAYS AND OTHER EXPENSIVE AND DEBILITATING LONG-TERM HEALTH AND SOCIAL PROBLEMS. SWOPE HEALTH SERVES A POPULATION WITH A HIGHER PREMATURITY RATE THAN THE NATIONAL AVERAGE. WE PROVIDE CASE MANAGEMENT AND RELATED SUPPORT SERVICES DESIGNED TO IDENTIFY, ENROLL AND KEEP PREGNANT WOMEN IN OUR PRENATAL PROGRAM. SWOPE HEALTH DATA SHOW THAT WOMEN IN OUR PRENATAL PROGRAM EXPERIENCE A LOWER INCIDENCE OF NEGATIVE OUTCOMES. FAMILY PLANNING SERVICES ARE ALSO OFFERED. KNOWING INADEQUATE DENTAL HYGIENE AND ORAL INFECTIONS CAN CONTRIBUTE TO PREMATURE LABOR, WE ALSO REFER PREGNANT WOMEN TO OUR DENTAL DEPARTMENT FOR ANY NECESSARY DENTAL CARE.SWOPE HEALTH ALSO SUPPORTS NEW MOTHERS WITH POST-PARTUM CARE THROUGH A VARIETY OF PROGRAMS AND SERVICES. FOR EXAMPLE, WE OFFER CLASSES AND ONE-TO-ONE COUNSELING TO ENCOURAGE WOMEN TO BREASTFEED THEIR BABIES, A STRATEGY THAT IS NOT ONLY COST SAVING BUT THAT PROVIDES A HOST OF LONG-TERM HEALTH BENEFITS TO BOTH THE MOTHER AND CHILD. OUR WIC PROGRAM PROVIDES BREAST PUMPS AND NUTRITION EDUCATION TO THESE NEW MOMS. SWOPE HEALTH HAS REALIGNED ASSETS AND RESOURCES FOR THE CARE OF INFANTS, CHILDREN AND ADOLESCENTS IN ITS DEPARTMENT OF CHILDREN'S SERVICES, WHICH SERVED NEARLY 9,000 CHILD AND ADOLESCENT PATIENTS IN 2019. SWOPE HEALTH HAS PROVIDED HEALTH CARE SERVICES TO TREAT EVERYTHING FROM SPRAINS AND CUTS TO COLDS AND THE FLU, AS WELL AS CHRONIC ILLNESS, INCLUDING ASTHMA AND BEHAVIORAL HEALTH CONDITIONS. CHILDREN'S SERVICES HAS DEVELOPED PROGRAMS AND COMMUNITY-BASED PARTNERSHIPS THAT IMPROVE ACCESS TO CARE AND SERVICES FOR VULNERABLE OR UNDERSERVED PEDIATRIC POPULATIONS, INCLUDING NEWBORN HEALTH SUPERVISION, ASTHMA MANAGEMENT, MOBILE PEDIATRIC SERVICES, AND COMMUNITY- AND SCHOOL-BASED SERVICES. BECAUSE THERE ARE SO MANY THINGS THAT CAN IMPAIR THE PHYSICAL, SOCIAL AND INTELLECTUAL DEVELOPMENT OF A CHILD, SWOPE HEALTH MAKES EVERY EFFORT TO IDENTIFY THESE POTENTIAL PROBLEMS EARLY AND INITIATE APPROPRIATE TREATMENT BEFORE A DEVELOPMENTAL PROBLEM BECOMES A PERMANENT DISABILITY.TO PROVIDE CHILDREN IN A HIGH-CRIME URBAN AREA WITH A SAFE AND EXCITING HALLOWEEN EXPERIENCE, SWOPE HEALTH SPONSORS AN ANNUAL "TREAT TOWN" AT ITS CENTRAL LOCATION. IN 2019, MORE THAN 1,500 CHILDREN ATTENDED THIS ANNUAL EVENT WHERE THEY RECEIVED TREATS, TOOTHBRUSHES AND OTHER IMPORTANT HEALTH INFORMATION, INCLUDING DETAILS ON HOW TO ACCESS DENTAL SERVICES AT OUR FACILITIES.ADULT MEDICAL SERVICES: SWOPE HEALTH ADVOCATES FOR PREVENTIVE CARE AND LONG-TERM MANAGEMENT OF CHRONIC CONDITIONS. THE MAJORITY OF SWOPE HEALTH PATIENTS HAVE CHRONIC CONDITIONS LIKE CARDIOVASCULAR DISEASE (HYPERTENSION, HYPERLIPIDEMIA OR MYOCARDIAL INJURY, TO NAME A FEW), DIABETES, DEPRESSION, ETC. IF NOT PROPERLY MANAGED, THESE CHRONIC CONDITIONS CAN HAVE SERIOUS AND LIFE-ALTERING COMPLICATIONS, SUCH AS HEART ATTACK, STROKE, AND KIDNEY FAILURE. OUR STATE-SPONSORED HEALTH CARE HOME PROGRAM INVOLVES CARE COORDINATORS AND CASE MANAGERS WORKING ONE-ON-ONE WITH PATIENTS TO HELP MANAGE THEIR HEALTH. NURSES IN THE HEALTH CARE HOME PROGRAM MONITOR THE CRITICAL INDICATORS OF THE PATIENTS' CHRONIC DISEASES; EXPLAIN THE IMPORTANCE OF ADHERING TO MEDICATION REGIMENS; AND ADVISE THEM ABOUT DIET, EXERCISE AND OTHER SELF-MANAGEMENT LIFESTYLE CHANGES THAT ARE KNOWN TO SLOW THE PROGRESSION OF THEIR DISEASE. CURRENTLY WE HAVE SEVERAL CASE MANAGERS/CARE COORDINATORS TO HELP OUR PATIENTS ACHIEVE THEIR HEALTH GOALS, AND WE HAVE HIRED STAFF TO PROVIDE SIMILAR CARE TO PATIENTS WITH CHRONIC DISEASE WHO ARE NOT ENROLLED IN THE HEALTH CARE HOME PROGRAM. WE HAVE MODIFIED OUR WORKFLOWS AND ORGANIZED OUR STAFF, INCLUDING CARE MANAGERS, TO WORK IN TEAMS TO BETTER HELP THESE PATIENTS NAVIGATE THE HEALTHCARE SYSTEM. THIS IS A HUGE INITIATIVE AND REQUIRES SEVERAL TASKS TRACKING LAB RESULTS, REFERRALS TO SPECIALTY CARE, HOSPITAL AND EMERGENCY ROOM FOLLOW UPS. WE CONTINUE TO HIRE CARE MANAGERS, REFERRAL COORDINATORS, AND COMMUNITY HEALTH WORKERS TO SUPPORT THIS INITIATIVE. SWOPE HEALTH ALSO HELPS PATIENTS WITH DIABETES AND OTHER CHRONIC DISEASES BY EDUCATING THEM ON DISEASE MANAGEMENT AND LIFESTYLE MODIFICATION. A DIABETES EDUCATOR, DIETICIAN, AND COMMUNITY HEALTH WORKERS SUPPORT THIS EFFORT. DENTAL SERVICES: ORAL HEALTH IS NECESSARY FOR OVERALL POSITIVE HEALTH OUTCOMES. PATIENTS WITH POOR ORAL HYGIENE ARE AT RISK FOR GUM DISEASE AND CAVITIES, AND STUDIES SHOW THESE PATIENTS ALSO ARE AT INCREASED RISK FOR HEART DISEASE AND STROKE. ADDITIONALLY, STUDIES HAVE SHOWN THAT DIABETIC PATIENTS WHO ALSO SUFFER FROM GUM DISEASE HAVE HIGHER HBA1C VALUES. ACCESS TO ADEQUATE DENTAL CARE IS DIFFICULT, PARTICULARLY FOR THE ADULT POPULATION. THIS IS DUE TO LIMITED COVERAGE OF DENTAL SERVICES BY PUBLIC INSURANCE PROGRAMS, SUCH AS MEDICAID AND MEDICARE.IN 2019, SWOPE HEALTH PROVIDED COMPREHENSIVE DENTAL CARE (SCREENINGS, EXAMINATIONS, X-RAYS, SEALANTS, FLUORIDE VARNISHES, RESTORATIONS, EXTRACTIONS, HYGIENE SERVICES INCLUDING TREATMENT OF GUM DISEASE, AND PROSTHETIC SERVICES INCLUDING PARTIALS, DENTURES, AND CROWN AND BRIDGE WORK) TO 8,600 ADULTS AND CHILDREN. WE OFFER WALK-IN APPOINTMENTS AND SAME DAY SERVICES FOR PATIENTS WITH PAIN AND DENTAL EMERGENCIES. WE COMMIT TO PROVIDE SAME-DAY TREATMENT TO RESOLVE ISSUES AND WE PRIORITIZE APPOINTMENTS FOR PEDIATRIC, OB, AND PATIENTS WITH CHRONIC CONDITIONS SUCH AS HIGH BLOOD PRESSURE OR DIABETES. IN 2019, WE INCREASED OUR MOBILE DENTAL SERVICES AND PROVIDED 889 VISITS. THIS INCLUDED VISITS TO 20 EARLY CHILDHOOD CENTERS WITH MID-AMERICA REGIONAL COUNCIL HEAD START FOR PATIENTS AGE THREE TO FIVE; ONE ELEMENTARY CHARTER SCHOOL AND KANSAS CITY PUBLIC SCHOOL CHILDREN KINDERGARTEN TO EIGHTH GRADES; SEVEN HOMELESS SHELTERS, INCLUDING MEN-ONLY SHELTERS, AND SHELTERS FOR WOMEN AND CHILDREN THAT INCLUDES A DOMESTIC VIOLENCE SHELTER. WE ALSO PROVIDE DENTAL SERVICES AT ONE-DAY EVENTS, SUCH AS THE PROJECT HOMELESS CONNECT OR BACK-TO-SCHOOL EVENTS, THROUGHOUT THE METROPOLITAN AREA INCLUDING KANSAS CITY, KS.BEHAVIORAL HEALTH SERVICES:IN 2019, A TOTAL OF 12,534 INDIVIDUALS RECEIVED COMPREHENSIVE PSYCHIATRIC SERVICES IN JACKSON COUNTY, MISSOURI, ACCORDING TO THE MISSOURI DEPARTMENT OF MENTAL HEALTH. ALTHOUGH THIS REPRESENTS JUST 2% OF THE POPULATION (703,211 IN MISSOURI, ACCORDING TO THE UNITED STATES CENSUS), THE GENERAL CONSENSUS IS THAT ROUGHLY 30% OF THE POPULATION HAS A MENTAL ILLNESS OR SUBSTANCE USE DISORDER. IT IS IMPORTANT TO NOTE THAT MORE THAN 12.5% OF THE POPULATION HAS NO HEALTH INSURANCE. SWOPE HEALTH BEHAVIORAL HEALTH PROVIDED SERVICES TO MORE THAN 5,500 INDIVIDUALS IN 2019. THOSE INDIVIDUALS RECEIVED MORE THAN 111,000 SERVICES, SOME WHICH OCCURRED ON THE SAME DAY. THESE SERVICES INCLUDE COMMUNITY SUPPORT, INDIVIDUAL AND GROUP THERAPY, MEDICATION ASSESSMENT AND MANAGEMENT, SUBSTANCE USE TREATMENT, RESIDENTIAL TREATMENT, CRISIS INTERVENTION, AND CASE MANAGEMENT. THESE SERVICES ARE AVAILABLE FOR AGES 3 AND OVER.
