Community Partnerships are decision-making entities, broadly representative of a county or multi-county area, that partner with state agencies to plan, develop, finance, and monitor strategies to achieve specific core results. Caring Communities is Missouri's system reform initiative to achieve the Core Resuls through Community Partnerships. Core Results: Children & Families Safe, Children & Families Healthy, Children Prepared to Enter School, Youth Ready to Become Productive Citizens, Parents Working. Our Strategy: Involve citizens in decisions which affect their well-being, bring services closer to families, use dollars more flexibly and effectively to meet the needs, be accountable for results.
With guidance from the Rentry Opportunity Center (ROC) staff, returning offenders assess their strengths and needs; then develop their own personal plan for successful transition back into the community. ROC staf assist the clients in locating and assessing appropriate resources to ensure success.
PCCP, MO Valley CAA, Habitat for Humanity, FSD, Circles of Support, Center for Human Services, MO Dept of Mental Health, Pathways Community Behavioral Health, University Behavioral Health, churches, businesses and individuals are working together to combat homelessness and provide housing assistance. The program includes case management for families utilizing PROP's Assistance.
